研究目的：

探讨围术期血糖波动对PELD术后患者预后的影响，分析血糖波动与术后疼痛缓解程度、并发症发生率以及患者生活质量改善等方面的相关性，为优化腰椎间盘突出症患者的围手术期血糖管理提供参考。  

Objectives of Study：

Explore the impact of perioperative blood glucose fluctuations on the prognosis of patients after PELD surgery, analyze the correlation between blood glucose fluctuations and postoperative pain relief, incidence of complications, as well as improvement in patients' quality of life, to provide a reference for optimizing perioperative blood glucose management in patients with lumbar disc herniation.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄18-75岁，确诊为单节段腰椎间盘突出； 2.符合PELD手术指征（影像学证实为腰椎间盘突出/保守治疗无效）； 3.术后随访≥3个月，且病历资料完整（含VAS、ODI评分）； 4.糖尿病患者：有明确 1 型 / 2 型糖尿病史（需提供病历或降糖药物使用记录），或术前空腹血糖≥7.0mmol/L 且糖化血红蛋白（HbA1c）≥6.5%；非糖尿病患者：无糖尿病史，术前空腹血糖＜6.1mmol/L 且 HbA1c＜5.7%。

Inclusion criteria

1.Age 18-75 years, diagnosed with single-segment lumbar disc herniation. 2.Meet the indications for Percutaneous Endoscopic Lumbar Discectomy (PELD) (confirmed by imaging as lumbar disc herniation / failure of conservative treatment). 3. Postoperative follow-up ≥ 3 months, with complete medical records (including Visual Analog Scale [VAS] and Oswestry Disability Index [ODI] scores). 4. Diabetic patients: have a documented history of type 1 or type 2 diabetes (requiring medical record or evidence of glucose-lowering medication use), OR have preoperative fasting plasma glucose ≥7.0 mmol/L AND glycated hemoglobin (HbA1c) ≥6.5%. Non-diabetic patients: have no history of diabetes, AND have preoperative fasting plasma glucose <6.1 mmol/L AND HbA1c <5.7%.

排除标准：

1.既往腰椎手术史； 2.有严重影响手术及预后的全身性疾病，如未控制的严重心肺功能衰竭、恶性肿瘤晚期、严重肝肾功能障碍等； 3.合并马尾神经损伤表现或合并脊柱骨折、肿瘤、感染及下肢关节病等非单纯椎间盘突出患者； 4.存在精神类疾病，如精神分裂症、严重的抑郁症等，导致患者无法配合术后随访及相关评估工作的患者； 5.妊娠期或哺乳期女性，近3个月内参与其他临床试验患者。

Exclusion criteria：

1. History of previous lumbar spine surgery. 2.Presence of systemic diseases that severely impact surgery and prognosis, such as uncontrolled severe cardiopulmonary failure, advanced malignant tumors, severe hepatic or renal dysfunction, etc. 3.Patients with manifestations of cauda equina syndrome, or those with concurrent spinal fractures, tumors, infections, or lower extremity joint diseases — in other words, patients whose condition is not solely attributed to disc herniation. 4. Presence of psychiatric disorders (e.g., schizophrenia, severe depression) that would compromise the patient's ability to cooperate with postoperative follow-up and relevant assessments. 5. Pregnant or lactating women, or patients who have participated in other clinical trials within the past 3 months.
