研究目的：

采用TCCD和TCD血流动力学不同参数对颈内动脉颅外段重度狭窄患者MCA狭窄程度及VA起始段重度狭窄患者颅内V4段狭窄程度进行超声分级评估，确定超声诊断标准，为临床治疗决策的选择提供参考依据。  

Objectives of Study：

Using different parameters of TCCD and TCD hemodynamics to evaluate the degree of MCA stenosis in patients with severe extracranial stenosis of the internal carotid artery and the degree of intracranial V4 stenosis in patients with severe stenosis of the VA initiation segment, ultrasound diagnostic criteria were determined to provide reference for clinical treatment decision-making.

纳入标准：

1.颈脑一体化超声诊断为颈内动脉颅外段重度狭窄或单侧椎动脉起始段重度狭窄的患者；
2.超声检查2周内进行支架置入手术，并进行DSA、CTA检查；
3.年龄>=18岁；
4.临床资料齐全；

Inclusion criteria

1.Patients diagnosed with severe stenosis of the extracranial segment of the internal carotid artery or severe stenosis of the initial segment of the unilateral vertebral artery using integrated neck brain ultrasound;
2.Perform stent placement surgery within 2 weeks of ultrasound examination, and perform DSA and CTA examinations;
3.Age >= 18 years old;
4.Clinical data is complete.

排除标准：

1.双侧颞窗不透声；
2.颈内动脉颅外段重度狭窄患者同侧MCA闭塞/同侧颈总动脉重度狭窄或闭塞；
3.颈内动脉颅外段重度狭窄患者对侧颈动脉重度狭窄；
4.单侧椎动脉起始段重度狭窄患者前循环重度狭窄或闭塞；
5.双侧锁骨下动脉重度狭窄或闭塞；
6.蛛网膜下腔出血等导致血管痉挛；
7.相关MCA或椎动脉溶栓或血管内治疗；
8.房颤或者心衰等影响速度测量的心律失常；
9.CT、MRI、DSA 检查禁忌症或拒绝检查；
10.严重出血性疾病或凝血功能障碍；
11.合并其他器官或组织严重疾病；

Exclusion criteria：

1.Bilateral temporal windows are not transparent to sound;
2.Patients with severe stenosis of the extracranial segment of the internal carotid artery have ipsilateral MCA occlusion/ipsilateral common carotid artery severe stenosis or occlusion;
3.Patients with severe stenosis of the extracranial segment of the internal carotid artery have severe stenosis of the contralateral carotid artery;
4.Patients with severe stenosis or occlusion of the anterior circulation in the initial segment of unilateral vertebral artery;
5.Severe stenosis or occlusion of bilateral subclavian arteries;
6.Vasospasm caused by subarachnoid hemorrhage, etc;
7.Related MCA or vertebral artery thrombolysis or endovascular treatment;
8.Arrhythmias such as atrial fibrillation or heart failure that affect velocity measurement;
9.CT, MRI, DSA examination contraindications or refusal of examination;
10.Severe bleeding disorders or coagulation dysfunction;
11.Merge serious diseases of other organs or tissues.
