研究目的：

本项目拟采用多维度研究设计框架：首先通过前瞻性队列研究进行病因探索，继而开展随机对照试验验证干预效果，最终结合自身前后对照试验实现个体化效应评估，系统解决以下关键科学问题： （1）内脏脂肪型肥胖和外周脂肪型肥胖患者是否有独特的肠道菌群和代谢物特点，以及其与心血管危险因素的相关性探讨； （2）探究不同类型生活方式干预对肥胖患者的脂肪分布、心血管危险因素、肠道菌群和代谢物的影响； （3）通过肥胖患者代谢减重手术前后的自身对照试验，验证心血管危险因素的改善是否伴随相应肠道菌群及代谢物的变化； （4）建立一套标准化改善肥胖症的生活方式减重方案；（5）为肥胖患者科学减重治疗、降低并发症风险提供新靶点和新视角。  

Objectives of Study：

This project plans to adopt a multidimensional research design framework: first, exploring etiology through a prospective cohort study; next, conducting randomized controlled trials to verify the effects of interventions; and finally, combining self-controlled trials to achieve individualized effect assessment, systematically addressing the following key scientific issues: (1) Whether patients with visceral obesity and peripheral obesity have unique gut microbiota and metabolite characteristics, and exploring their correlation with cardiovascular risk factors; (2) Investigating the effects of different types of lifestyle interventions on fat distribution, cardiovascular risk factors, gut microbiota, and metabolites in obese patients; (3) Through self-controlled trials before and after metabolic weight-loss surgery in obese patients, verifying whether improvements in cardiovascular risk factors are accompanied by corresponding changes in gut microbiota and metabolites; (4) Establishing a standardized lifestyle weight-loss program for improving obesity; (5) Providing new targets and perspectives for the scientific weight-loss treatment of obese patients and for reducing the risk of complications.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄18-50岁； 2.24kg/m^2=90cm，女性>=85cm，腰臀比男性>=0.9，女性>=0.85 ; （2）双能X线：内脏脂肪型腿部脂肪量（SFA）占总脂肪量百分比<40% ; （3）CT/MRI：将在第二腰椎中心VFA>100cm2定义为内脏脂肪型肥胖； 4.周围脂肪型肥胖：符合年龄18-50岁，24kg/m^2

Inclusion criteria

1. Age 18-50 years old; 2. 24kg/m^2 < BMI < 40kg/m^2; 3. Visceral fat-type obesity: (1) Waist circumference: men >= 90cm, women >= 85cm; waist-to-hip ratio: men >= 0.9, women >= 0.85; (2) Dual-energy X-ray: leg fat mass of visceral fat-type (SFA) accounts for less than 40% of total fat mass; (3) CT/MRI: VFA at the center of the second lumbar vertebra > 100 cm^2 is defined as visceral fat-type obesity; 4. Peripheral fat-type obesity: meets age 18-50 years and 24kg/m^2 < BMI < 40kg/m^2, but does not meet the diagnostic criteria for visceral fat-type obesity; 5. There is more than a 25% difference in VFA and SFA between the two groups.

排除标准：

1.已确诊糖尿病、心脏病或高血压、胃肠道疾病者；
2.每周不能参加运动>75分钟的高强度运动或>150分钟的中等强度运动者；
3.体重在近3月出现明显变化；
4.过去3个月内使用过抗生素，并持续3天或以上；
5.妊娠期以及哺乳期妇女；
6.酗酒（1周饮酒5次以上，平均每次2两白酒或半斤黄酒或5瓶啤酒以上）；
7.过去6个月内患有严重的精神疾患；
8.过去3个月内因下列疾病接受药物治疗的：胆囊炎、消化道溃疡、尿路感染、急性肾盂肾炎、膀胱炎、甲亢等甲状腺功能异常；
9.进行过胃肠道手术，除阑尾炎、疝气手术外；
10.慢性、迁延性肝炎患者或肝硬化等严重肝脏疾病患者，和乙肝病毒表面抗原（HBsAg）阳性并伴随有肝功能有异常者（血清谷丙转氨酶和谷草转氨酶是正常值的2.5倍）；
11.患有IBD、库欣综合征；
12.患有严重器质性疾病如癌症、冠心病、心肌梗塞以及中风等；
13.患有肺结核和艾滋病等传染病；
14.肢体残疾，或其他原因引起的生活不能自理、不能清楚地回忆、回答问题者；
15.贫血：血红蛋白<10g/dl；
16.过去3月内以任何药物方法（包括氟苯丙氨等食欲抑制剂类药物和甲状腺激素、孕酮、泻剂等和各种中药成分减肥药）进行减肥者；
17.有明显的运动障碍；
18.不能保证充足的时间参与本项目的；

Exclusion criteria：

1.Those who have been diagnosed with diabetes, heart disease or hypertension, or gastrointestinal diseases.
2.Those who cannot participate in more than 75 minutes of high-intensity exercise or more than 150 minutes of moderate-intensity exercise per week.
3.There has been a noticeable change in weight over the past three months.
4.Used antibiotics in the past three months for three days or more.
5.Pregnant and breastfeeding women.
6.Heavy drinking (drinking alcohol five or more times a week, with an average of more than 2 liang of white spirit, half a jin of yellow wine, or five bottles of beer per occasion.
7.Suffered from a severe mental illness in the past six months.
8.Medications were taken for the following conditions in the past three months: cholecystitis, peptic ulcers, urinary tract infections, acute pyelonephritis, cystitis, hyperthyroidism, and other thyroid dysfunctions.
9.Underwent gastrointestinal surgery, excluding appendectomies and hernia repairs.
10.Patients with chronic or persistent hepatitis, or severe liver diseases such as cirrhosis, and those who are HBsAg positive with abnormal liver function (serum ALT and AST levels are 2.5 times the normal values).
11.Suffering from IBD, Cushing's syndrome.
12.Suffering from severe organic diseases such as cancer, coronary heart disease, myocardial infarction, and stroke.
13.Suffer from infectious diseases such as tuberculosis and AIDS.
14.Those with physical disabilities or other reasons that prevent them from taking care of themselves, clearly recalling or answering questions.
15.Anemia: Hemoglobin <10g/dl.
16.Individuals who have used any medication (including appetite suppressants such as fenfluramine, thyroid hormones, progesterone, laxatives, and various traditional Chinese medicine weight loss drugs) for weight loss in the past three months.
17.There are obvious motor impairments.
18.There is no guarantee of sufficient time to participate in this project.
