研究目的:

1.评价在日喀则真实临床场景下,远程机器人辅助TKA在术后下肢力线恢复精度方面是否不劣于上海专家在日喀则现场实施的机器人辅助TKA。 2.评价两组在围术期安全性、手术过程指标、早中期功能评分及患者满意度方面的差异。 3.基于上海第九人民医院2025年成熟中心现场机器人辅助TKA历史数据库,评价日喀则远程机器人辅助TKA的关键质量指标是否接近上海第九人民医院水平。  

Objectives of Study:

1. To evaluate whether remote robot-assisted TKA is non-inferior to on-site robot-assisted TKA performed by Shanghai experts in Shigatse in terms of the accuracy of postoperative lower limb alignment restoration under real clinical scenarios in Shigatse. 2. To compare the two groups in perioperative safety, intraoperative procedural parameters, early and mid-term functional scores, and patient satisfaction. 3. Based on the historical database of mature on-site robot-assisted TKA cases at Shanghai Ninth People’s Hospital in 2025, to evaluate whether the key quality indicators of remote robot-assisted TKA in Shigatse are comparable to those of Shanghai Ninth People’s Hospital.

纳入标准:

1.年龄18岁以上,80岁以下。 2.临床及影像学诊断为膝骨关节炎,符合初次单侧TKA适应证。 3. 拟接受机器人辅助全膝关节置换术。 4. 能够理解研究内容并签署知情同意书。 5. 能够按要求完成术后影像学检查和随访。

Inclusion criteria

1. Age over 18 years and under 80 years. 2. Clinically and radiographically diagnosed with knee osteoarthritis, meeting the indications for initial unilateral TKA. 3. Planning to undergo robot-assisted total knee arthroplasty. 4. Able to understand the study content and sign the informed consent form. 5. Able to complete postoperative imaging examinations and follow-up as required.

排除标准:

1.翻修TKA或双侧同期TKA。 2. 严重关节外畸形需额外截骨矫形者。 3. 活动性感染、严重凝血障碍或其他手术禁忌证。 4. 既往同侧膝关节重大手术史明显影响标准化测量者。 5. 不能完成随访或影像评估者。 6. 研究者判断不适合入组者。

Exclusion criteria:

1. Revision TKA or bilateral simultaneous TKA. 2. Severe extra-articular deformity requiring additional osteotomy for correction. 3. Active infection, severe coagulation disorders, or other surgical contraindications. 4. History of major surgery on the same knee significantly affecting standardized measurements. 5. Unable to complete follow-up or imaging assessments. 6. Subjects deemed unsuitable for inclusion by the investigator.

由上海专家完成远程术前规划。 术中上海专家通过远程控制终端实时操控机器人机械臂完成关键骨切/定位步骤。 日喀则本地医生负责术野暴露、软组织处理配合、假体安放及切口关闭。 采用统一机器人平台、统一假体系统、统一围术期管理流程。

Intervention:

Preoperative surgical planning was completed remotely by Shanghai expert surgeons. During surgery, Shanghai experts manipulated the robotic arm in real time via a remote control terminal to perform key bone resection and positioning procedures. Local surgeons in Shigatse were responsible for surgical field exposure, soft tissue management and coordination, prosthesis implantation, and wound closure. A unified robotic platform, standardized prosthesis system, and consistent perioperative management protocol were adopted in all cases.

Preoperative planning and intraoperative robotic procedures were completed on-site in Shigatse by Shanghai expert surgeons. The robotic platform, prosthesis system, anesthesia protocol, perioperative management, and rehabilitation pathway remained identical to those in the experimental group.

主RCT采用个体1:1区组随机,由独立统计人员预先生成随机序列,区组长度建议为4或6,使用密封不透明信封或电子随机系统实施分配隐藏。 采用术前冠状位畸形程度和BMI作为分层因素;将协作医生作为协变量纳入统计校正。

Randomization Procedure (please state who generates the random number sequence and by what method):

The main RCT adopts individual 1:1 block randomization. The random sequence is pre-generated by an independent statistician with a recommended block size of 4 or 6. Allocation concealment is implemented using sealed opaque envelopes or an electronic randomization system. Preoperative coronal deformity severity and BMI are adopted as stratification factors; collaborating physicians are included as a covariate for statistical adjustment.
