研究目的：

构建科学严谨的早产儿母亲群组育儿保健方案，验证早产儿母亲群组育儿保健方案在早产儿母亲育儿及健康管理中的可行性和有效性。  

Objectives of Study：

Develop a scientifically rigorous Group well-child care for mothers of premature infants, and validate the feasibility and effectiveness of the Group well-child care for mothers of premature infants in parenting and health management.

纳入标准：

质性研究： 1.因早产（32≤孕周≤36+6）其早产儿需要接受住院治疗的母亲； 2.意识清楚、语言表达能力正常； 3.年龄≥18岁； 4.自愿参与本项目的调查。 可行性研究： 1.于32≤孕周≤36+6周分娩的产妇； 2.其新生儿出生后24小时内转入NICU者。

Inclusion criteria

Qualitative Research: 1. Mothers whose premature infants (with gestational weeks ranging from 32 to 36+6) require inpatient treatment; 2. Those with clear consciousness and normal language expression ability; 3. Those aged 18 years or older; 4. Those who voluntarily participate in the investigation of this project. Feasibility Study: 1. Parturients whose babies were born within the gestational weeks range of 32 to 36+6; 2. Those whose newborns were transferred to the NICU within 24 hours after birth.

排除标准：

质性研究： 1.存在沟通障碍； 2.患有严重精神或躯体疾病； 3.其早产儿有先天性畸形、遗传性疾病或其他严重并发症。 可行性研究： 1. 早产儿母亲患有严重心、脑、肺、肾等脏器疾病或其他严重慢性疾病； 2. 母亲存在语言交流障碍； 3. 母亲分娩后转入重症监护病房治疗者； 4. 早产儿放弃治疗； 5. 早产儿患有先天性遗传代谢病、先天畸形及其他需要进行外科手术的疾病。

Exclusion criteria：

Qualitative Research: 1. There are communication barriers; 2. Suffer from severe mental or physical illnesses; 3. Their premature infants have congenital deformities, genetic diseases or other serious complications. Feasibility Study: 1. The mother of the premature infant has severe heart, brain, lung, kidney, or other organ diseases, or other serious chronic diseases; 2. The mother has language communication difficulties; 3. The mother was transferred to the intensive care unit for treatment after giving birth; 4. Treatment of the premature infant was abandoned; 5. The premature infant has congenital genetic metabolic diseases, congenital malformations, or other diseases requiring surgical intervention.

对照组实施常规出院指导、电话随访、门诊随访。出院时责任护士对早产儿母亲进行口头健康教育发放健康手册。告知出院后注意事项和产后42天来院进行复诊，出院后社区护士对早产儿母亲进行随访。在早产儿出院当天向早产儿母亲或家属进行出院宣教，包括居家环境、喂养知识、疾病预防、出院带药、复诊等。早产儿出院随访：在早产儿出院第1、第3个月后分别进行电话随访，询问早产儿喂养、生长发育、预防接种等情况，进行答疑解惑，并提醒按时来院复查。

The control group received routine discharge instructions, telephone follow-ups, and outpatient follow-ups. At discharge, the responsible nurse provided oral health education to the mothers of preterm infants and distributed health manuals. They were informed of post-discharge precautions and instructed to return to the hospital for a follow-up visit 42 days postpartum. Community nurses conducted follow-ups with the mothers of preterm infants after discharge. On the day of discharge, the mothers or family members of preterm infants were provided with discharge education, including information on home environment, feeding knowledge, disease prevention, medications to take home, and follow-up visits. Post-discharge follow-up for preterm infants: Telephone follow-ups were conducted one and three months after discharge to inquire about feeding, growth and development, and vaccination status, address any concerns, and remind them to return to the hospital for follow-up visits on time.

一、 干预实施流程 每次群组活动包含三个环节： (1) 健康评估 针对当期母婴护理需求进行评估（如生命体征、乳房、伤口、恶露等），帮助母亲了解自身健康状态并参与决策。 (2) 小组讨论 围绕当期主题，采用PPT、模型、角色扮演、情景模拟等方式开展互动式学习，引导母亲分享经验、主动提问。五期主题依次为： 第1期：早期母乳喂养管理与情绪管理 第2期：早产儿照护（感染预防、日常观察、人工喂养） 第3期：早产儿异常问题管理（腹胀、大便异常、皮肤问题、呛奶、哭闹） 第4期：产后康复（哺乳期营养、盆底肌及腹直肌锻炼） 第5期：早产儿早期发展指导（睡眠、喂养、交流玩耍） (3) 小组练习 通过模具进行操作演练，干预者提供个性化指导，鼓励家属共同参与。

I. Intervention Implementation Process Each group activity consists of three phases: (1) Health Assessment An assessment is conducted based on the current maternal and infant care needs (such as vital signs, breasts, wounds, lochia, etc.), helping the mother understand her own health status and participate in decision-making. (2) Group Discussion Interactive learning is carried out using PPT, models, role-playing, and scenario simulation around the current theme. The mother is guided to share experiences and ask questions actively. The five themes are as follows: Phase 1: Early Breastfeeding Management and Emotional Management Phase 2: Care for Premature Infants (Infection Prevention, Daily Observation, Artificial Feeding) Phase 3: Management of Premature Infant Abnormalities (Abdominal Distension, Abnormal Feces, Skin Problems, Aspiration, Crying) Phase 4: Postpartum Rehabilitation (Nutrition during Lactation, Pelvic Floor Muscle and Abdominal Rectus Muscle Exercises) Phase 5: Early Development Guidance for Premature Infants (Sleeping, Feeding, Communication and Play) (3) Group Practice Operational drills are conducted using molds, with personalized guidance provided by the interventionists and encouraging family members to participate together.
