研究目的：

针对由下颌第三磨牙组生阻生所导致的下颌第二磨牙远中颈部龋，以传统II类洞充填修复技术为对照，研究牙周-牙体联合治疗方法的有效性与安全性。为临床处理颇为棘手的下颌第二磨牙远中颈部龋提供新的治疗策略和有效的治疗方法。  

Objectives of Study：

To investigate the effectiveness and safety of the combined periodontal-restorative treatment technique for distal cervical caries of the mandibular second molar caused by impaction mandibular third molar, using traditional Class II cavity restoration technique as a control. To provide new treatment strategies and effective treatment methods for the difficult clinical treatment of caries in the distal neck of the mandibular second molar.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.患者年龄限定18～65 周岁； 2. 至少有一侧下颌第二磨牙（M2M）存在由阻生第三磨牙（M3M）导致的远中颈部龋（DCC），且M3M已拔除 ≥ 4周； 3. 患牙远中边缘嵴完整（影像学及临床检查确认）； 4. 患牙牙髓活力正常（冷测+电活力测试）； 5. 患者理解研究内容并签署知情同意书。

Inclusion criteria

1. Aged 18-65 years; 2. At least one mandibular second molar with deep subgingival distal cervical caries caused by an impacted third molar, and the impacted third molar has been extracted for >=4 weeks; 3. Intact distal marginal ridge of the affected tooth; 4. Normal pulp vitality of the affected tooth; 5. Patients who understand the study and voluntarily sign informed consent.

排除标准：

1. M2M DCC龋洞冠方边缘至远中边缘嵴的最短距离 < 3mm（影像学测量）； 2. M2M DCC龋洞龈壁位于釉牙骨质界根方 ≤ 2mm 或 ≥ 5mm（翻瓣后或影像学估算）； 3. M2M牙髓活力异常、已诊断为牙髓炎或根尖周炎； 4. 未经控制的牙周炎（全口PLI > 20%，BI > 2，或有 > 2颗牙PD ≥ 5mm 且伴BOP）； 5. 张口度≤ 25mm； 6. 存在未控制的严重全身系统性疾病（如未控制糖尿病、免疫缺陷病、凝血功能障碍）或局麻禁忌证； 7. 严重骨质疏松（骨密度T值≤-2.5）或长期使用双膦酸盐类药物者 8. 已怀孕、哺乳期或计划在2年内怀孕； 9. 无法配合完成治疗或随访； 10. 研究者判断不适合参与研究。

Exclusion criteria：

1. Distance from coronal margin of the cavity to distal marginal ridge < 3mm; 2. Gingival wall of the cavity located <= 2mm or >= 5mm apical to the cementoenamel junction; 3. Abnormal pulp vitality, diagnosed pulpitis or periapical periodontitis; 4. Uncontrolled periodontitis; 5. Mouth opening <= 25mm; 6. Uncontrolled severe systemic diseases or contraindications to local anesthesia; 7. Severe osteoporosis or long-term bisphosphonate use; 8. Pregnancy, lactation, or planned pregnancy within 2 years; 9. Inability to comply with treatment or follow-up; 10. Investigator's judgment of inappropriateness for participation.
