研究目的：

本研究旨在通过腹腔镜中低位直肠癌术中早期结肠镜下吻合口钛夹封闭技术，与传统预防性回肠造口方案开展对照研究，明确该技术对吻合口漏、吻合口出血的预防作用，降低术后吻合口漏发生率，减少中低位直肠癌手术预防性造口比例，避免造口相关并发症与二次手术创伤，减轻患者经济负担，改善术后生活质量。同时建立规范、安全、可推广的临床操作流程，为结直肠外科临床提供更微创、高效的吻合口防护方案，完善相关临床研究证据，提升区域结直肠疾病诊疗水平与社会效益。  

Objectives of Study：

The aim of this study is to compare the use of titanium clip closure technology for anastomosis during early colonoscopy in low rectal cancer surgery with traditional prophylactic ileostomy methods. The aim is to clarify the preventive effect of this technology on anastomotic leakage and bleeding, reduce the incidence of postoperative anastomotic leakage, decrease the proportion of prophylactic ostomy in low rectal cancer surgery, avoid stoma related complications and secondary surgical trauma, reduce the economic burden on patients, and improve postoperative quality of life. At the same time, establish standardized, safe, and scalable clinical operating procedures to provide more minimally invasive and efficient anastomotic protection solutions for colorectal surgery, improve relevant clinical research evidence, and enhance the diagnosis and treatment level and social benefits of regional colorectal diseases.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.经肠镜 + 病理确诊，肿瘤下缘距肛缘 5.1~10 cm（中位）、0~5 cm（低位）； 2.行腹腔镜中低位直肠癌根治术，R0 切除； 3.存在≥1 项 AL 高危因素（糖尿病、肥胖、肿瘤 T3~T4、低蛋白、新辅助放化疗后、吻合口距肛缘＜5 cm）； 4.临床资料完整，随访≥3 个月。

Inclusion criteria

1. Confirmed by colonoscopy and pathology, the distance between the lower edge of the tumor and the anal margin is 5.1-10 cm (median) and 0-5 cm (low); 2. Perform laparoscopic low rectal cancer radical resection with R0 resection; 3. There is more than one high risk factor of AL (diabetes, obesity, tumor T3~T4, low protein, after neoadjuvant radiotherapy and chemotherapy, anastomotic stoma<5 cm from the anal margin); 4. Complete clinical data and follow-up of >= 3 months.

排除标准：

1.严重心肝肾不全、凝血障碍； 2.术前肠梗阻需急诊手术； 3.造口或钛夹禁忌； 4.失访。

Exclusion criteria：

1. Severe liver, liver and kidney insufficiency, coagulation disorders; 2. Preoperative intestinal obstruction requires emergency surgery; 3. Taboo of stoma or titanium clip; 4. Lost to follow-up.

The random sequence was generated by an independent research assistant who did not participate in patient screening and surgical operations using the SPSS 26.0 statistical software's random number generation program.
