通过前瞻性案例集设计，评估噬菌体治疗耐药细菌感染患者的安全性和有效性。  

Objectives of Study：

To evaluate the safety and efficacy of phage therapy in patients with drug-resistant urinary tract infections using a prospective case-series design

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄18-75周岁（包含临界值），性别不限； 2.耐药细菌感染患者（以本院或其他三甲医院药敏检测结果为据）； 3.患者感染的病原菌应满足如下条件之一： （1）病原菌对关键抗生素耐药： 1）鲍曼不动杆菌：碳青霉烯耐药； 2）肠杆菌属（如肺炎克雷伯菌、大肠埃希菌）：碳青霉烯或头孢菌素耐药； 3）铜绿假单胞菌：碳青霉烯耐药； 4）肠球菌：万古霉素耐药； 5）金黄色葡萄球菌：甲氧西林或万古霉素耐药； 6)嗜麦芽窄食单胞菌。 *参考 WHO Bacterial Priority Pathogens List, 2024 （2）病原菌体外对关键抗生素敏感但此抗生素治疗无效或效果不明显（患者靶细菌反复感染7天及以上而期间使用抗生素治疗无法清除）； （3）病原菌对关键敏感抗生素使用受限（如器官毒性或对此抗生素过敏患者）； 4.能够筛选到至少一种裂解性噬菌体； 5.能够通过感染灶喷洒、外敷、雾化吸入、灌注、局部注射或导流管等方式给药； 6.受试者及家属（或监护人）充分阅读、理解并签署知情同意书。

Inclusion criteria

1. Age 18-75 years (including the boundary values), no gender restriction; 2. Patients with drug-resistant bacterial infections (based on antibiotic susceptibility test results from this hospital or other tertiary hospitals); 3. The pathogenic bacteria infecting the patient should meet at least one of the following conditions: (1) The pathogen is resistant to key antibiotics: 1) Acinetobacter baumannii: resistant to carbapenems; 2) Enterobacteriaceae (such as Klebsiella pneumoniae, Escherichia coli): resistant to carbapenems or cephalosporins; 3) Pseudomonas aeruginosa: resistant to carbapenems; 4) Enterococcus: resistant to vancomycin; 5) Staphylococcus aureus: resistant to methicillin or vancomycin; 6) Stenotrophomonas maltophilia. *Reference WHO Bacterial Priority Pathogens List, 2024 (2) The pathogen is sensitive to key antibiotics in vitro but the treatment with these antibiotics is ineffective or the effect is not obvious (patients with recurrent infection by target bacteria for 7 days or more during which antibiotic treatment fails to clear the infection); (3) The use of key sensitive antibiotics is limited for the pathogen (e.g., organ toxicity or allergy to the antibiotic); 4. At least one lytic bacteriophage can be screened; 5. Administration is possible via infection site spraying, external application, nebulized inhalation, perfusion, local injection, or drainage tubes; 6. The subject and family members (or guardians) have fully read, understood, and signed the informed consent form.

排除标准：

1.细菌感染能被抗生素治疗有效控制；
2.存在未解除的外科情况，如未引流的脓肿或梗阻等；
3.合并心/肝/肺/肾等脏器功能衰竭的患者；
4.患有严重免疫性疾病的患者；
5.本次噬菌体给药方式无法有效触达耐药菌的相关疾病；
6.患者感染的细菌现有噬菌体种类无法覆盖的；
7.除静脉注射给药外无其他给药途径；
8.经研究者判定其他不适合参与本研究的患者。

Exclusion criteria：

1.Bacterial infection that can be effectively controlled by antibiotic therapy.
2.Unresolved surgical conditions, such as undrained abscess or obstruction.
3.Presence of heart, liver, lung, kidney or other vital organ failure.
4.Patients with severe immunological disorders.
5.Conditions in which the planned phage administration route cannot effectively reach the drug-resistant bacteria.
6.No available phage(s) cover the patient’s infecting bacterial strain.
7.No feasible administration route other than intravenous injection.
8.Other conditions judged by the investigator to be inappropriate for study participation.
