研究目的：

本研究通过对PC-DM与PC-NDM患者癌旁胰腺组织的免疫荧光染色和定量分析，旨在比较两组患者的基本临床病理特征，验证其基线可比性，并通过临床指标与病理特征的相关性分析，试图从免疫、凋亡、淀粉样沉积三个维度揭示胰腺癌相关糖尿病的潜在发病机制。  

Objectives of Study：

This study, through immunofluorescence staining and quantitative analysis of peritumoral pancreatic tissues from patients with PC-DM and PC-NDM, aims to compare the basic clinicopathological characteristics of the two groups, verify their baseline comparability, and, via correlation analysis between clinical indicators and pathological features, attempt to elucidate the potential pathogenesis of pancreatic cancer-associated diabetes mellitus from three dimensions: immunity, apoptosis, and amyloid deposition.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.疾病诊断：经术后病理学检查证实为胰腺导管腺癌，AJCC第8版分期为Ⅰ期或Ⅱ期，并且拥有完整的临床病理资料； 2.标本可用性：拥有福尔马林固定、石蜡包埋（FFPE）的肿瘤组织及癌旁组织（距肿瘤边缘>2cm）标本，且数量和质量足以完成所有计划的病理学检测； 3.分组特定标准：PC-DM组（实验组）：在胰腺导管腺癌确诊前3年内有明确的糖尿病诊断记录（符合WHO糖尿病诊断标准：典型糖尿病症状加上随机血糖≥11.1 mmol/L，或加上空腹血糖≥7.0 mmol/L，或加上OGTT 2h血糖≥11.1 mmol/L，或加上糖化血红蛋白HbA1c≥6.5%）；PC-NDM组（对照组）：无糖尿病病史。术前空腹血糖< 6.1 mmol/L且HbA1c < 5.7%； 4.数据完整性：拥有本研究所需的关键临床数据，包括但不限于年龄、性别、BMI、肿瘤分期、术前血清CA19-9水平、糖尿病相关治疗史等。

Inclusion criteria

1. Disease diagnosis:Histopathologically confirmed pancreatic ductal adenocarcinoma (PDAC) after surgery, classified as stage I or II according to the AJCC 8th edition, with complete clinicopathological data. 2. Specimen availability:Availability of formalin-fixed, paraffin-embedded (FFPE) tumor tissue and adjacent non?tumor tissue (distance from tumor margin >2 cm), with sufficient quantity and quality to complete all planned pathological assays. 3. Group?specific criteria:PC?DM group (experimental group):A definite diagnosis of diabetes mellitus (DM) recorded within 3 years prior to the diagnosis of PDAC, meeting the WHO diagnostic criteria for diabetes (typical diabetic symptoms plus random plasma glucose >=11.1 mmol/L, or fasting plasma glucose >=7.0 mmol/L, or 2?hour plasma glucose during OGTT >=11.1 mmol/L, or HbA1c >=6.5%). PC?NDM group (control group):No history of diabetes mellitus, with preoperative fasting plasma glucose <6.1 mmol/L and HbA1c <5.7%. 4. Data completeness:Availability of essential clinical data required for this study, including but not limited to age, sex, BMI, tumor stage, preoperative serum CA19?9 level, and history of diabetes?related treatment.

排除标准：

1.其他类型的糖尿病，如1型糖尿病、妊娠糖尿病； 2.术前接受过新辅助化疗或者放疗，这有可能对组织形态产生影响； 3.合并其他胰腺疾病，如慢性胰腺炎、IPMN； 4.受到全身性疾病干扰，如合并其他已知的恶性肿瘤、患有严重的肝、肾功能不全； 5.数据与标本质量不过关，像临床资料严重缺失或者不完整，没办法进行准确分组或者分析的情况，以及病理组织标本经过评估质量太差或者数量不足，不能满足实验要求的； 6.病理诊断为胰腺恶性肿瘤，像腺鳞癌、胶样癌、神经内分泌肿瘤等； 7.癌症分期过晚，即病理分期为Ⅲ期或IV期的患者。

Exclusion criteria：

1. Other types of diabetes mellitus, such as type 1 diabetes mellitus or gestational diabetes mellitus. 2. Receipt of neoadjuvant chemotherapy or radiotherapy before surgery, which may affect tissue morphology. 3. Concomitant other pancreatic diseases, such as chronic pancreatitis or intraductal papillary mucinous neoplasm (IPMN). 4. Interference by systemic diseases, such as concomitant other known malignant tumors, or severe hepatic or renal insufficiency. 5. Poor quality of data or specimens, e.g., severely missing or incomplete clinical data that preclude accurate grouping or analysis, or pathological tissue specimens deemed of insufficient quality or quantity to meet experimental requirements. 6. Pathological diagnosis of other pancreatic malignancies, such as adenosquamous carcinoma, colloid carcinoma, or neuroendocrine tumor. 7. Advanced cancer stage, i.e., patients with pathological stage III or IV.
