阐明慢病多病共存人群可治疗特征的分布规律与共现模式，基于特征数据进行聚类分析识别典型亚型，构建针对不同亚型的差异化分层管理策略建议。  

Objectives of Study：

Elucidate the distribution patterns and co-occurrence modes of treatable traits in the population with multimorbidity of chronic diseases, identify typical subtypes through clusteralysis based on trait data, and construct differentiated stratified management strategy recommendations for different subtypes.

纳入标准：

1.南石头街社区卫生服务中心在管居民； 2.年龄≥35岁； 3.患有至少2种慢性病（包括但不限于：高血压、糖尿病、慢阻肺、冠心病、脑卒中、骨质疏松等）； 4.有完整的可治疗特征评估数据（基于TTs慢病管理系统，包含部分可以后期补充数据）。

Inclusion criteria

1. Residents under the management of Nanshitou Street Community Health Service Center; 2. Age >= 35 years old; 3. Suffering from at least two chronic diseases (including but not limited to: hypertension, diabetes, chronic obstructive pulmonary disease, coronary heart disease, stroke, osteoporosis, etc.); 4. Having complete assessable data on treatable characteristics (based on the TTs chronic disease management system, including some data that can be supplemented later).

排除标准：

1..关键变量数据缺失＞20%； 2. 预期生存时间＜6个月（如晚期肿瘤等）。

Exclusion criteria：

1. Missing data for key variables > 20%; 2. Expected survival time < 6 months (such as advanced tumors).
