研究目的:

本课题旨在以NLRP3炎症小体激活和内皮糖萼破坏为核心,系统探讨儿童单肺通气诱导肺损伤的机制。通过从炎症反应和血管内皮屏障损伤的双重角度揭示单肺通气引发的肺损伤过程,明确关键分子和信号通路,寻找潜在的干预靶点,为减少单肺通气相关肺损伤提供理论基础和临床依据。  

Objectives of Study:

This project aims to systematically investigate the mechanisms of one-lung ventilation–induced lung injury in children, with a particular focus on NLRP3 inflammasome activation and endothelial glycocalyx disruption. By elucidating the pathophysiological processes of lung injury triggered by one-lung ventilation from the dual perspectives of inflammatory responses and vascular endothelial barrier dysfunction, this study seeks to identify key molecules and signaling pathways involved, as well as potential therapeutic targets. Ultimately, the findings are expected to provide a theoretical foundation and clinical evidence for the prevention and mitigation of one-lung ventilation–associated lung injury.

纳入标准:

选择 2026 年 4 月 1 日至 2028 年 4 月 1 日。接受择期单肺通气胸腔镜手术的患儿 250 例

Inclusion criteria

Select the period from April 1, 2026 to April 1, 2028. A total of 250 children underwent elective thoracoscopic surgery with one-lung ventilation

排除标准:

1. 术前患有肺炎或其他呼吸功能异常; 2. 术前影像学检查提示肺部炎症、渗出、不张等; 3. 存在严重心血管疾病、免疫缺陷或其他影响研究指标的疾病; 4. 术中不耐受单肺通气改双肺通气者。

Exclusion criteria:

1. Preoperative pneumonia or other abnormalities in respiratory function; 2. Preoperative imaging findings suggestive of pulmonary inflammation, exudation, or atelectasis; 3. Presence of severe cardiovascular disease, immunodeficiency, or other conditions that may affect the study outcomes; 4. Intraoperative intolerance to one-lung ventilation requiring conversion to two-lung ventilation.
