研究目的:

本项目拟通过多模态MRI(3D-T1WI、DTI、动态弥散成像dDWI),构建适用于脑积水患儿的脑结构、室管膜结构及脑脊液的流体力学的量化方法,系统定量地刻画和解读脑积水脑结构异常与脑脊液的流体力学表征的关联机制,并以此为基础,探究用于区分AH与进展性脑积水的关键生物学标志物与鉴别诊断方案。  

Objectives of Study:

This project aims to develop quantitative methodologies based on multimodal MRI (3D-T1-weighted imaging , diffusion tensor imaging , and dynamic diffusion-weighted imaging ) to characterize brain structure, ependymal architecture, and cerebrospinal fluid hydrodynamics in pediatric hydrocephalus. It will systematically quantify and elucidate the mechanistic associations between structural brain abnormalities and cerebrospinal fluid hydrodynamic features. Building upon these analyses, the study further seeks to identify key biological markers and establish robust diagnostic strategies for differentiating arrested hydrocephalus from progressive hydrocephalus.

纳入标准:

病例组纳入标准: (1)根据国际最新脑积水guideline诊断标准,Evan's≥0.33; (2)年龄:0岁~3岁; (3)性别不限。 对照组纳入标准: (1)性别与参加脑影像学研究的病例组匹配,年龄与患者匹配,年龄差异≤1岁。 (2)未患有精神心理疾病及任何严重躯体疾病。 (3)常规MRI检查正常。

Inclusion criteria

Case group inclusion criteria: (1) According to the latest international hydrocephalus guideline diagnostic criteria, Evan's>=0.33; (2) Age: 0 to 3 years old; (3) No gender limit. Inclusion criteria for control group: (1) Gender was matched to the case group participating in the brain imaging study, and age was matched to the patient, with an age difference of <=1 year. (2) No mental illness or any serious physical disease. (3) Routine MRI examination was normal.

排除标准:

1.患有其他精神疾病(如精神分裂症等); 2.患有遗传代谢性疾病; 3.患有癫痫等严重神经疾病以及颅脑损伤史等重大躯体疾病史。 4.常规MRI检查异常。

Exclusion criteria:

1. Suffering from other mental disorders (such as schizophrenia, etc.); 2. Have inherited metabolic diseases; 3. Have a history of severe neurological diseases such as epilepsy and major physical diseases such as craniocerebral injury. 4. Abnormal results of routine MRI examination.

Target condition (The target condition is a particular disease or disease stage that the index test will be intended to identify. Please specify the characteristics in detail; the population should has a complete spectrum and good representative):

(1) Our target population comprises children aged 0–3 years diagnosed with hydrocephalus in accordance with the latest international guideline criteria; (2) age- and sex-matched typically developing healthy children.

容易混淆的疾病人群(即与目标疾病不易区分的一种或多种不同疾病,应避免采用正常人群对照的病例-对照设计):

良性外部脑积水、脑萎缩、先天性畸形及围产期脑损伤在影像学上可类似脑积水,但可通过临床病史及多模态MRI加以鉴别。

Population with condition difficult to distinguish from the target condition, the normal population in a case-control study design should be avoid:

Benign external hydrocephalus, brain atrophy, congenital malformations, and perinatal brain injury may mimic hydrocephalus on imaging but can be distinguished by clinical history and multimodal MRI.
