研究目的:

1、探讨高压氧联合骨形态发生蛋白-2早期无创干预青壮年股骨颈骨折内固定术的临床疗效。 2、探讨高压氧、骨形态发生蛋白-2联合地舒单抗早期无创干预骨质疏松性股骨颈骨折内固定术的临床疗效。  

Objectives of Study:

1. To explore the clinical efficacy of early non-invasive intervention with hyperbaric oxygen combined with bone morphogenetic protein-2 in internal fixation surgery for femoral neck fractures in young and middle-aged adults. 2. To explore the clinical efficacy of early non-invasive intervention with hyperbaric oxygen and bone morphogenetic protein-2 combined with denosumab in internal fixation surgery for osteoporotic femoral neck fractures.

Description for medicine or protocol of treatment in detail:

 

纳入标准:

1.年龄 18-60 岁2.单侧股骨颈骨折3.骨折受伤时间小于 2 周4.既往无股骨颈骨折病史。

Inclusion criteria

1.Aged 18–60 years; 2. Unilateral femoral neck fracture 3. Time from injury less than 2 weeks;4.No previous history of femoral neck fracture.

排除标准:

1.陈旧性股骨颈骨折; 2.病理性骨折; 3.合并有类风湿性骨关节炎、先天性髋关节发育不良或髋关节股骨头坏死者; 4.合并有其他严重的代谢性疾病; 5.长期酗酒、激素使用者。

Exclusion criteria:

1. Old femoral neck fracture; 2. Pathological fracture; 3. Patients with rheumatoid arthritis, congenital hip dysplasia, or avascular necrosis of the femoral head; 4. Patients with other severe metabolic diseases; 5. Long-term alcohol abusers or steroid users.

数据采集和管理(说明:数据采集和管理由两部分组成,一为病例记录表(Case Record Form, CRF),二为电子采集和管理系统(Electronic Data Capture, EDC),如ResMan即为一种基于互联网的EDC:

本研究采用病例记录表(CRF)结合电子数据采集系统(EDC)进行数据采集和管理。CRF依据研究方案设计,涵盖受试者基线资料、手术相关指标、干预措施执行情况、影像学数据、实验室数据及随访结局指标(骨折愈合时间、骨折不愈合率、股骨头坏死率、Harris髋关节功能评分、VAS评分等)。采用EDC系统(ResMan公共管理平台,www.medresman.org)实现数据的电子化录入和管理,EDC系统具备结构化数据录入、逻辑核查、稽查痕迹、权限分级管理等功能。制定CRF填写指南(CCG),所有数据录入人员均接受统一培训。数据核查包括系统自动核查和人工核查,对存疑数据通过质疑管理流程进行处理。受试者个人信息进行去标识化处理,确保数据安全与隐私保护。研究结束后,完整数据集将导出归档,原始数据通过ResMan平台提供,供审核和公示。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture:

combined with an Electronic Data Capture (EDC) system. The CRF is designed according to the study protocol and covers baseline characteristics of subjects, surgical parameters, implementation of interventions, imaging data, laboratory data, and follow-up outcome measures (fracture healing time, fracture nonunion rate, femoral head necrosis rate, Harris Hip Score, VAS score, etc.). An EDC system (ResMan Public Management Platform, www.medresman.org) is used for electronic data entry and management. The EDC system features structured data entry, logical checks, audit trails, and role?based access control. A CRF Completion Guideline (CCG) is developed, and all personnel involved in data entry receive standardized training. Data verification includes both automated system checks and manual checks, with queries on questionable data being handled through a query resolution process. Subjects' personal information is de?identified to ensure data security and privacy protection. Upon completion of the study, the complete dataset will be exported and archived, and the raw data will be made available through the ResMan platform for review and public disclosure.
