研究目的：

探索膝关节腔内注射异体人源脂肪间充质干细胞外泌体 (Adipose Tissue Mesenchymal Stem Cells-derived Exosomes，AD-MSCs-Exo) 治疗膝骨关节炎的安全性、耐受性和初步的有效性。  

Objectives of Study：

To explore the safety, tolerability and preliminary efficacy of intra-articular injection of allogeneic human adipose tissue mesenchymal stem cell-derived exosomes (AD-MSCs-Exo) in the treatment of knee osteoarthritis.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.诊断为膝骨关节炎的患者（依据中华医学会骨科学分会《2021年骨关节炎诊疗指南》）；
2.Kellgren-Lawrence分级为Ⅱ~Ⅲ；
3.年龄在18-75岁；
4.VAS疼痛评分（Visual Analog Scale）4分以上，且WOMAC疼痛分量表评分≥2分；
5.目标膝关节有持续至少3个月的慢性疼痛，或经物理/药物治疗3个月效果不佳、无法耐受或无法缓解的；
6.愿意遵守治疗计划并签署知情同意书；

Inclusion criteria

1.Patients diagnosed with knee osteoarthritis (according to the "2021 Osteoarthritis Diagnosis and Treatment Guidelines" by the Orthopaedic Society of the Chinese Medical Association); 2.Kellgren-Lawrence grade II to III; 3.Age between 18-75 years old; 4.VAS pain score (Visual Analog Scale) of 4 or above, and WOMAC pain score >= 2 points; 5.The target knee joint has chronic pain that lasts for at least 3 months, or has poor, intolerable, or unresolvable effects after 3 months of physical/medication treatment; 6.Willing to follow the treatment plan and sign the informed consent form;

排除标准：

1.急性炎症期； 2.BMI>40； 3.曾行关节置换术或关节镜手术； 4.合并有其他关节疾病或导致膝关节疼痛的疾病：如类风湿性关节炎、化脓性关节炎、强直性脊柱炎症、前交叉韧带撕裂、半月板撕裂、绒毛结节性滑膜炎、滑膜软骨瘤病、创伤性关节炎、感染性疾病等； 5.合并有全身性疾病：如未控制的严重系统性疾病、肿瘤性疾病等； 6.4 周内行膝关节腔内类固醇注射治疗； 7.过去一年内曾接受干细胞等生物治疗； 8.过去一年内服用全身免疫抑制药物； 9.3 个月内服用抗风湿性疾病药物（包括甲氨蝶呤或其他抗代谢药物）； 10.肺结核、艾滋病毒、肝炎和梅毒等传染病； 11.不明原因的发热>38.0 ℃； 12.酒精、药物滥用史； 13.中重度焦虑、抑郁等心理疾病； 14.已知对研究药物任何成分过敏； 15.不理解普通话，不认识汉字。

Exclusion criteria：

1. Acute inflammatory phase; 2. BMI > 40; 3. History of joint replacement surgery or arthroscopic surgery; 4. Presence of other joint diseases or diseases causing knee pain, such as rheumatoid arthritis, suppurative arthritis, ankylosing spondylitis, anterior cruciate ligament tear, meniscus tear, villonodular synovitis, synovial chondromatosis, traumatic arthritis, infectious diseases, etc.; 5. Presence of systemic diseases, such as uncontrolled severe systemic diseases, tumors, etc.; 6. Intra-articular steroid injection in the knee within 4 weeks; 7. Received stem cell or other biological treatments within the past year; 8. Systemic immunosuppressive medication use within the past year; 9. Use of antirheumatic drugs within the past 3 months (including methotrexate or other antimetabolic drugs); 10. Infectious diseases such as pulmonary tuberculosis, HIV, hepatitis, and syphilis; 11. Fever of unknown origin > 38.0 ℃; 12. History of alcohol or drug abuse; 13. Moderate to severe anxiety, depression, or other psychological disorders; 14. Known allergy to any component of the study drug; 15. Does not understand Mandarin or cannot recognize Chinese characters.
