研究目的：

本研究旨在通过回顾性分析过去 12 年间 45 岁及以上住院患者的临床数据，比较伴与不伴精神病性症状的重度抑郁发作患者在 PSG 参数、炎症标志物、代谢指标、甲状腺功能及性激素水平上的差异，探索可用于辅助鉴别诊断的客观生物学指标。  

Objectives of Study：

This study aims to retrospectively analyze clinical data from inpatients aged 45 years and older over the past 12 years, compare differences in PSG parameters, inflammatory markers, metabolic indicators, thyroid function, and sex hormone levels between patients with major depressive episode with psychotic symptoms and those without, and explore objective biological indicators that can be used to assist in differential diagnosis.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. 年龄≥45 周岁，性别不限； 2. 2014 年 1 月 1 日至 2025 年 12 月 31 日期间在北京大学第六医院老年科或睡眠科住院治疗； 3. 符合 ICD-10 重度抑郁发作诊断标准，具体编码包括： （1）F32.2 不伴有精神病性症状的重度抑郁发作 （2）F32.3 伴有精神病性症状的重度抑郁发作 （3）F33.2 复发性抑郁障碍，目前为不伴有精神病性症状的重度发作 （4）F33.3 复发性抑郁障碍，目前为伴有精神病性症状的重度发作； 4. 具有完整的住院病历记录，能够提取研究所需的核心变量信息。

Inclusion criteria

1. Age >= 45 years, with no gender restriction; 2. Inpatients admitted to the Department of Geriatric Psychiatry or the Department of Sleep Medicine, Peking University Sixth Hospital, between January 1, 2014 and December 31, 2025; 3. Meeting the ICD?10 diagnostic criteria for major depressive episode, with specific codes including: (1) F32.2 Major depressive episode, single episode, without psychotic symptoms; (2) F32.3 Major depressive episode, single episode, with psychotic symptoms; (3) F33.2 Recurrent depressive disorder, current episode severe without psychotic symptoms; (4) F33.3 Recurrent depressive disorder, current episode severe with psychotic symptoms; 4. Having complete inpatient medical records that allow extraction of core variable information required for the study.

排除标准：

1. 睡眠障碍：中-重度睡眠呼吸暂停（AHI≥15 次/min）、快动眼睡眠行为障碍； 2. 精神障碍：精神分裂症谱系障碍、双相情感障碍、恶劣心境； 3. 神经系统疾病：痴呆、帕金森病/综合征、癫痫、迟发性运动障碍； 4. 系统性疾病：干燥综合征、类风湿关节炎、系统性红斑狼疮、慢性肝炎、肝硬化、肾病综合征、支气管哮喘、慢性萎缩性胃炎、恶性肿瘤； 5. 内分泌疾病：任何临床甲状腺功能减退症（包括原发性、手术后）/亢进； 6. 急性事件：近 1 个月内感染（发热、感冒、肺炎等）、近 6 个月内急性心脑血管事件（急性脑梗死、不稳定性心绞痛、心肌梗死）、近 6 个月内重大手术； 7. 数据不完整：关键变量（分组变量或主要结局指标）缺失且无法通过原始病历补充者。

Exclusion criteria：

1. Sleep disorders:Moderate-to-severe sleep apnea (AHI >= 15 times/min), rapid eye movement sleep behavior disorder; 2. Psychiatric disorders:Schizophrenia spectrum disorders, bipolar disorder, dysthymia; 3. Neurological diseases:Dementia, Parkinson’s disease/syndrome, epilepsy, tardive dyskinesia; 4. Systemic diseases:Sj?gren’s syndrome, rheumatoid arthritis, systemic lupus erythematosus, chronic hepatitis, liver cirrhosis, nephrotic syndrome, bronchial asthma, chronic atrophic gastritis, malignant neoplasms; 5. Endocrine diseases:Any clinical hypothyroidism (including primary and postsurgical) or hyperthyroidism; 6. Acute events:Infections within the past 1 month (fever, cold, pneumonia, etc.), acute cardiovascular and cerebrovascular events within the past 6 months (acute cerebral infarction, unstable angina pectoris, myocardial infarction), major surgery within the past 6 months; 7. Incomplete data:Patients with missing key variables (grouping variables or primary outcome measures) that cannot be supplemented from the original medical records.
