本研究旨在评估高龄（>=35岁）且有胚胎移植失败史或首次移植失败、仍有冻存囊胚的患者中，采用无创染色体筛查（NICS）与形态学结合的胚胎优选策略的临床效果。具体目标包括： 评估NICS与形态学联合策略的效果：采用“解冻—短时复苏培养—NICS检测—复冻”策略，对高龄患者的冻融胚胎进行染色体评估，并结合形态学评分进行胚胎选择。比较“形态学”与“NICS+形态学”策略对妊娠结局的影响，包括持续妊娠率、临床妊娠率、流产率等。 优化高龄患者冻存胚胎的优选路径：提供无创筛查方法，减少胚胎侵入性操作，提高胚胎选择精准性，减少非整倍体胚胎移植风险，优化妊娠结局。 提供真实世界循证依据：为高龄患者提供NICS与形态学联合评估冻存胚胎选择的证据，推动NICS技术的临床应用。 探索NICS的临床价值：通过大规模、多中心的前瞻性研究，评估NICS在不同临床条件下的应用效果，为高龄IVF患者提供更科学合理的胚胎筛选方案，尤其是针对冻存囊胚的患者。  

Objectives of Study：

Evaluate the effectiveness of the NICS and morphology combined strategy: The strategy of “thawing—short-term recovery culture—NICS testing—refreezing” will be used to assess the chromosomal status of embryos in elderly patients before frozen-thawed embryo transfer (FET), combined with morphological grading for embryo selection. The impact of the “morphology” and “NICS + morphology” strategies on pregnancy outcomes, including sustained pregnancy rate, clinical pregnancy rate, miscarriage rate, etc., will be compared.Optimize embryo selection pathways for elderly patients with cryopreserved embryos: Provide a non-invasive screening method to reduce embryo-invasive procedures, improve embryo selection accuracy, reduce the risk of aneuploid embryo transfer, and ultimately optimize pregnancy outcomes.Provide real-world evidence for clinical practice: Provide reliable evidence for selecting cryopreserved embryos based on NICS and morphological assessment without embryo biopsy, promoting the clinical application of NICS technology, especially its effectiveness and feasibility in real-world clinical settings.Explore the clinical value of NICS as an embryo selection method: Through a large-scale, multicenter prospective study, assess the application of NICS under different clinical conditions and provide a new, scientifically reasonable embryo screening approach for elderly IVF patients, especially for those with cryopreserved blastocysts.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. 接受 ICSI 或 IVF 辅助生殖技术的不孕症夫妇，不包含受卵/供精周期； 2. 女性年龄>=35 岁（以取卵日为准）； 3. 有胚胎移植（鲜胚/冻胚）失败（未妊娠/生化妊娠/流产/胎停）史，包含 1-2 次移植失败或流产 1 次且流产物染色体检测为阳性。 4. 女性体重指数（BMI）在 18-30 kg/m^2 之间； 5. 剩余冻存的囊胚数>=2 枚且囊胚天数均为 D5/D6；或者有冻存的卵裂胚养囊胚后，总检测囊胚数>=2 枚；囊胚评级需 4BC/4CB 及以上； 6. 入组后首次移植是单囊胚移植； 7. 夫妻染色体核型正常。

Inclusion criteria

1. Infertile couples who receive ICSI or IVF assisted reproductive technology, excluding egg/sperm donation cycles; 2. Female age >=35 years old (based on the date of egg retrieval); 3. There is a history of failed embryo transfer (fresh embryo/frozen embryo) (non-pregnancy/biochemical pregnancy/miscarriage/fetal arrest), including 1-2 failed transfers or 1 miscarriage, and the chromosomal test of the miscarriage product is positive. 4. The female body mass index (BMI) is between 18 and 30 kg/m^2. 5. The number of remaining frozen blastocysts is >=2 and the blastocyst days are all D5/D6. Or, after culturing blastocysts from frozen cleaved embryos, the total number of blastocysts tested is >=2. The blastocyst rating should be 4BC/4CB or above. 6. The first transfer after enrollment was single blastocyst transfer; 7. The karyotypes of the couple are normal.

排除标准：

1. 不适合进行 IVF 或 ICSI 的疾病，如肝病、肾病、I 型或 II 型糖尿病、心脏病、贫血、控制不良的高血压、脑血管事件病史、深静脉血栓和/或肺栓塞病史、宫颈癌、子宫内膜癌或乳腺癌病史以及未诊断的阴道出血； 2. 任何既往周期做过或计划做植入前遗传学检测/筛查者：PGT-A/PGT-M/PGT-SR（含本次拟做）或既往已对本批胚胎做过 NICS； 3. 可能影响妊娠结局的病理情况，包括生殖器畸形、输卵管积水、宫内感染、直径>4 厘米的子宫肌瘤、直径>4 厘米的盆腔或腹腔良性肿瘤、子宫内膜厚度<7 毫米且无法改善、垂体肿瘤和恶性肿瘤； 4. 未治疗的高泌乳素血症、甲状腺疾病和肾上腺疾病； 5. 在解冻移植周期前未治疗的子宫内膜息肉； 6. 反复种植失败（经过三个新鲜或冷冻周期至少移植三个优质胚胎后，未能临床怀孕）及反复流产（20 周前发生两次或以上连续流产）患者。

Exclusion criteria：

1. Diseases that are not suitable for IVF or ICSI, such as liver disease, kidney disease, type 1 or type 2 diabetes, heart disease, anemia, poorly controlled hypertension, history of cerebrovascular events, history of deep vein thrombosis and/or pulmonary embolism, history of cervical cancer, endometrial cancer or breast cancer, and undiagnosed vaginal bleeding; 2. Anyone who has undergone or plans to undergo preimplantation genetic testing/screening in previous cycles: PGT-A/PGT-M/PGT-SR (including those to be conducted this time) or has undergone NICS for this batch of embryos in the past; 3. Pathological conditions that may affect pregnancy outcomes, including genital malformations, hydrosalpinx, intrauterine infections, uterine fibroids with a diameter greater than 4 cm, benign pelvic or abdominal tumors with a diameter greater than 4 cm, endometrial thickness less than 7 mm that cannot be improved, pituitary tumors and malignant tumors; 4. Untreated hyperprolactinemia, thyroid diseases and adrenal diseases; 5. Endometrial polyps that were not treated before the thawing and transplantation cycle; 6. Patients with repeated implantation failure (failure to achieve clinical pregnancy after at least three high-quality embryos have been transplanted in three fresh or frozen cycles) and those with repeated miscarriage (two or more consecutive miscarriages before 20 weeks).
