本研究拟开展前瞻性随机对照临床试验,比较导板配准与牙面配准在机器人辅助种植手术中三维定位精度、术前准备效率和操作便捷性方面的表现,以验证牙面配准在适宜牙列条件下的临床可行性及应用价值,为机器人种植的标准化推广提供科学依据。  

Objectives of Study:

This study intends to conduct a prospective randomized controlled clinical trial to compare the performance of guide plate alignment and tooth surface alignment in terms of three-dimensional positioning accuracy, preoperative preparation efficiency, and operational convenience in robotic-assisted implant surgery. The aim is to verify the clinical feasibility and application value of tooth surface alignment under suitable dental arch conditions, and to provide a scientific basis for the standardized promotion of robotic implantation.

Description for medicine or protocol of treatment in detail:

 

纳入标准:

1.年龄>=18岁,性别不限,无全身系统性疾病(如未控制的糖尿病、严重骨质疏松症等); 2.计划接受种植修复的牙列缺损患者; 3.缺牙区邻近存在至少三颗无松动度的天然牙,缺牙区同牙列对侧同样存在至少三颗无松动度的天然牙; 4.口腔卫生状况良好,全口菌斑指数<=25%,全口出血指数<=25%; 5.患者自愿参加本研究,理解并签署知情同意书。

Inclusion criteria

1. Age >= 18 years old, gender not restricted, no systemic diseases throughout the body (such as uncontrolled diabetes, severe osteoporosis, etc.); 2. Patients with missing teeth who plan to undergo implant restoration; 3. The area of missing teeth is adjacent to at least three natural teeth without mobility, and the same side of the opposite dental row also has at least three natural teeth without mobility; 4. Good oral hygiene, the plaque index of the entire mouth <= 25%, the bleeding index of the entire mouth <= 25%; 5. Patients voluntarily participate in this study, understand and sign the informed consent form.

排除标准:

1.重度吸烟者(每日吸烟量>10支);
2.有长期服用影响骨代谢或伤口愈合药物史(如双膦酸盐类药物、免疫抑制剂、糖皮质激素等);
3.妊娠期或哺乳期妇女;
4.过去5年内有头颈部恶性肿瘤放疗史或正在接受化疗者;
5.存在开口度过小(<40mm)或其它张口受限、颞下颌关节弹响、关节疼痛等颞下颌关节问题者。

Exclusion criteria:

1. Heavy smokers (>10 cigarettes per day); 2. Long-term use of drugs affecting bone metabolism or wound healing (such as bisphosphonates, immunosuppressants, glucocorticoids, etc.); 3. Pregnant or lactating women; 4. Patients with a history of radiotherapy for head and neck cancer within the past 5 years or undergoing chemotherapy; 5. Patients with small mouth opening (<40mm) or other temporomandibular joint problems such as limited mouth opening, clicking and joint pain.

Intervention:

The personalized guide plates, 3D printed based on the patient's CBCT and intraoral scanning data, are used for intraoperative registration. The actual fabrication of the personalized guide plates is carried out using medical-grade photopolymer 3D printers (such as DLP or SLA systems) to ensure high precision and excellent forming quality. The printing material is medical-grade, biocompatible photo-sensitive resin, which complies with the standards for oral surgery guide plates.

本研究采用分层区组随机化以保证组间可比性并控制关键预后因素。以“缺牙区域”(前牙/后牙区)为分层因素,因其可能影响手术精度。随机序列由独立统计人员通过计算机生成,每层内按区组长度4或6以1:1比例分配至导板配准组或牙面配准组。分配结果存入密封编号信封或中心随机系统,仅在患者入组并签署知情同意后由主刀医生开启,以实现分配隐藏。

Randomization Procedure (please state who generates the random number sequence and by what method):

This study used stratified block randomization to ensure comparability between groups and control key prognostic factors. “Tooth loss region” (anterior/posterior) was used as the stratification factor, as it may affect surgical precision. The randomization sequence was generated by an independent statistician using a computer, with blocks of 4 or 6 within each stratum and a 1:1 allocation to either the guide-registration group or the tooth-surface registration group. Allocation results were kept in sealed, numbered envelopes or a central randomization system and were only revealed to the surgeon after patient enrollment and informed consent, ensuring allocation concealment.
