研究目的：

通过测量患者术中局部脑氧饱和度绝对变化值、术后炎性因子表达水平和简明精神状态量表MMSE评分，比较七氟烷吸入麻醉和丙泊酚全凭静脉麻醉两种不同的麻醉方法，对非急性期脆弱脑功能老年患者术后早期认知功能的影响；同时比较两种麻醉方式安全性、麻醉恢复时间、患者满意度等，为此类高危人群临床麻醉的选择及其对术后认知功能的恢复提供可靠证据。  

Objectives of Study：

By measuring the absolute change value of intraoperative regional cerebral oxygen saturation, postoperative inflammatory factor expression level and Mini-Mental State Examination (MMSE) score, this study compared the effects of two different anesthesia methods, sevoflurane inhalation anesthesia and propofol total intravenous anesthesia, on early postoperative cognitive function in elderly patients with non-acute fragile brain function; meanwhile, the safety, anesthesia recovery time and patient satisfaction of the two anesthesia methods were compared, so as to provide reliable clinical evidence for the selection of clinical anesthesia and the recovery of postoperative cognitive function in such high-risk populations.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄 >= 65岁、ASA分级Ⅱ-Ⅲ 级、拟行腹腔镜胃肠手术患者； 2.术前简单精神状态量表不低于23分； 3.有脑出血、脑梗死、TIA、脑血管狭窄、PD、长期头疼头晕等病史，但近三个月未发作且无相应的临床症状。

Inclusion criteria

1. Patients aged >= 65 years, ASA classification II-III, scheduled for laparoscopic gastrointestinal surgery; 2. Preoperative Mini-Mental State Examination score not less than 23; 3. History of cerebral hemorrhage, cerebral infarction, TIA, cerebrovascular stenosis, Parkinson's disease, chronic headaches or dizziness, but no episodes in the past three months and no corresponding clinical symptoms.

排除标准：

1.合并有颅内高压的患者； 2.合并有严重系统性疾病的患者； 3.重度贫血患者； 4.二次手术患者； 5.不能正常沟通的患者； 6.前额有外伤或感染的患者。

Exclusion criteria：

1. Patients with intracranial hypertension; 2. Patients with severe systemic diseases; 3. Patients with severe anemia; 4. Patients undergoing secondary surgery; 5. Patients who cannot communicate normally; 6. Patients with trauma or infection on the forehead.
