研究目的：

本研究旨在系统评估常规定位方法与超声定位方法在膈神经电刺激中的应用效果，为临床无创膈神经电刺激的超声定位方法提供依据。  

Objectives of Study：

This study aims to systematically evaluate the application effects of conventional positioning methods and ultrasound positioning methods in phrenic nerve electrical stimulation, providing a basis for the ultrasound positioning method of non-invasive phrenic nerve electrical stimulation in clinical practice.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄 ≥18岁； 2.接受MV； 3.BMI<30 kg/m2； 4.签署知情同意书。

Inclusion criteria

1. Age >= 18 years old; 2. Receiving mechanical ventilation (MV); 3. BMI < 30 kg/m2; 4. Signed informed consent form.

排除标准：

1.存在膈神经损伤或功能障碍风险的患者（如颈部手术史、神经肌肉疾病、严重COPD等）； 2.植入心脏起搏器、严重心律失常、心脏功能不全（NYHA分级III-IV级）患者 ； 3.BMI > 30 kg/m2； 4.孕妇或哺乳期妇女； 5.多器官功能衰竭患者； 6.气胸； 7.活动性结核患者。

Exclusion criteria：

1. Patients with a risk of diaphragmatic nerve injury or dysfunction (such as those with a history of neck surgery, neuromuscular diseases, severe COPD, etc.) ; 2. Patients with implanted cardiac pacemakers, severe arrhythmias, or cardiac dysfunction (NYHA classification III-IV) ; 3. BMI > 30 kg/m2 ; 4. Pregnant or lactating women ; 5. Patients with multiple organ failure ; 6. Pneumothorax ; 7. Active tuberculosis patients.

采用计算机生成的随机数字表法对受试者进行随机分配，将其分为两种干预顺序： （1）常规定位 → 超声引导定位序列； （2）超声引导定位 → 常规定位序列。 随机序列由研究人员使用计算机随机数生成，按照随机顺序确定每位受试者接受两种干预的先后顺序。

Randomization Procedure (please state who generates the random number sequence and by what method)：

Participants will be randomly assigned to one of two intervention sequences using a computer-generated randomization list: (1) Standard positioning followed by ultrasound-guided positioning; (2) Ultrasound-guided positioning followed by standard positioning. The random sequence determines the order in which each participant receives the two interventions.
