研究目的：

构建并验证一个面向胰腺多病种（包括胰腺癌、急/慢性胰腺炎、胰腺囊性疾病等）的专用CT影像基础模型（Pancreas-ViT），并系统评估该模型在多个下游诊断任务中的性能与临床效用。  

Objectives of Study：

Construct and validate a dedicated CT imaging foundation model (Pancreas-ViT) for multiple pancreatic diseases (including pancreatic cancer, acute/chronic pancreatitis, pancreatic cystic diseases, etc.), and systematically evaluate the model's performance and clinical utility across multiple downstream diagnostic tasks.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄 ≥ 18周岁，性别不限。 2.已完成或计划进行符合研究要求的胰腺相关腹部CT检查（包括平扫和/或增强扫描）。 3.因疑似或确诊胰腺疾病（如胰腺炎、胰腺肿瘤、胰腺囊性病变等）就诊，或属于胰腺癌高危筛查对象。 4.具备可追溯的完整临床病历、实验室检查及随访资料。 5.若参与特定任务研究（如胰腺癌可切除性评估），需为已确诊或高度疑似胰腺癌且拟行手术治疗的患者。

Inclusion criteria

1. Age >= 18 years, any gender. 2. Have completed or are scheduled to undergo research?compliant abdominal CT examination related to the pancreas (including non?contrast and/or contrast?enhanced scans). 3. Seeking medical care for suspected or confirmed pancreatic diseases (e.g., pancreatitis, pancreatic tumors, pancreatic cystic lesions, etc.), or belong to a high?risk screening population for pancreatic cancer. 4. Have traceable complete clinical records, laboratory test results, and follow?up data. 5. If participating in specific task studies (e.g., resectability assessment of pancreatic cancer), must be patients with confirmed or highly suspected pancreatic cancer who are planned to undergo surgical treatment.

排除标准：

1.CT图像存在严重伪影、运动模糊或扫描范围不全，影响胰腺评估。 2.曾接受过胰腺部分或全部切除手术。 3.伴有广泛腹腔转移、大量腹水或严重肠气干扰等影响胰腺观察的腹部疾病。 4.处于妊娠或哺乳期。 5.因精神障碍、认知功能受损等原因无法配合完成研究流程。 6.目前正在参与其他可能干扰本研究的临床试验。 7.存在碘对比剂过敏、未控制的甲状腺功能亢进等增强CT扫描禁忌证。

Exclusion criteria：

1. Severe artifacts, motion blur, or incomplete scanning range on CT images that affect pancreatic assessment. 2. Prior partial or total pancreatectomy. 3. Concurrent abdominal diseases that interfere with pancreatic observation, such as extensive peritoneal metastases, massive ascites, or severe bowel gas interference. 4. Pregnancy or lactation. 5. Inability to comply with the study procedures due to mental disorders, cognitive impairment, or other reasons. 6. Currently participating in another clinical trial that may interfere with this study. 7. Contraindications to contrast?enhanced CT, including allergy to iodinated contrast media or uncontrolled hyperthyroidism.

金标准或参考标准（即可准确诊断某疾病的单项方法或多项联合方法，在本研究中用于诊断是否有该病的临床参考标准）：

手术/活检病例：以术后组织病理学诊断作为最终金标准。 未手术病例：由多学科专家委员会依据影像、临床资料及≥2年影像随访结果形成共识诊断。 对照组：健康对照需CT正常且≥1年随访无胰腺疾病；非胰腺疾病对照需胰腺形态密度正常且随访无异常。

Gold Standard or Reference Standard (The clinical reference standards required to establish the presence or absence of the target condition in the tested population in present study):

Surgical/biopsy cases: Postoperative histopathological diagnosis serves as the final gold standard. Non-surgical cases: Consensus diagnosis is established by a multidisciplinary expert committee based on imaging, clinical data, and at least 2 years of imaging follow?up results. Control group: Healthy controls require normal CT findings and at least 1 year of follow?up without pancreatic disease; non?pancreatic disease controls require normal pancreatic morphology and density with no abnormalities during follow?up.
