研究目的：

探讨并构建维持性血液透析患者太极柔力球运动的方案。通过对白榕主编2008版《太极柔力球》以及段全伟主编2009版《太极柔力球教程》的学习，根据《我国成人血液透析患者康复治疗的专家共识》、《血液透析室（中心）肾脏康复体系建设的专家共识》以及《慢性肾脏病患者功能评估及康复服务规范》，制定一套符合透析患者的太极柔力球运动方案，并开展专家会议为干预方案提供参考意见。招募自2023年3月～2023年6月收治于昆山市第一人民医院的76名维持性血液透析患者，随机分为两组，为对照组和试验组各38名。干预前后分别对患者自体动静脉内瘘侧上肢进行握力、捏力（拇指-食指，拇指-中指，拇指-无名指）、腕关节活动度（主动掌曲、背伸）、以及肩关节活动度（内旋、外旋和外展）的测量，以及自体动静脉内瘘侧上肢肌电图（桡侧腕屈肌、旋前圆肌）RMS值的测量，与内瘘吻合口自然血流量和动脉化的静脉血管直径的测量，来评价干预方案的可行性及初步效果。  

Objectives of Study：

Explore and construct a exercise plan for Tai Chi softball for maintenance hemodialysis patients. Through the study of "Tai Chi Soft Force Ball" edited by Bai Rong in 2008 and "Tai Chi Soft Force Ball Tutorial" edited by Duan Quanwei in 2009, in accordance with the "Expert Consensus on Rehabilitation Treatment for Adult Hemodialysis Patients in China", "Expert Consensus on the Construction of Renal Rehabilitation System in Hemodialysis Rooms (Centers)", and "Standards for Functional Assessment and Rehabilitation Services for Patients with Chronic Kidney Disease", Develop a set of Tai Chi softball exercise plans suitable for dialysis patients and hold expert meetings to provide reference opinions for the intervention plan. A total of 76 maintenance hemodialysis patients admitted to the First People's Hospital of Kunshan from March 2023 to June 2023 were recruited and randomly divided into two groups: the control group and the experimental group, with 38 patients in each group. Before and after the intervention, the grip strength, pinching strength (thumb - index finger, thumb - middle finger, thumb - ring finger), wrist joint range of motion (active palm-flexion, dorsiflexion), and shoulder joint range of motion (internal rotation, external rotation, and abduction) of the upper limb on the side of the autologous arteriovenous fistula of the patients were measured respectively, as well as the RMS values of the electromyography (radial wrist flexor muscle, pronator teres muscle) of the upper limb on the side of the autologous arteriovenous fistula. The measurement of the natural blood flow at the anastomotic site of the internal fistula and the diameter of the arterialized vein was conducted to evaluate the feasibility and initial effect of the intervention plan.

纳入标准：

1. 上肢自体动静脉内瘘患者； 2. 接受血液透析治疗时间>=3个月； 3. 认知功能正常，沟通交流无障碍者； 4. 知情同意，愿意接受并配合本研究的实施。

Inclusion criteria

1. Patients with autologous arteriovenous fistula in the upper limb; 2. The duration of hemodialysis treatment is >=3 months; 3. Those with normal cognitive function and no communication barriers; 4. Informed consent, willing to accept and cooperate with the implementation of this study.

排除标准：

1. 使用其他血管通路进行透析的患者； 2. 生活无法自理者； 3. 精神障碍、认知障碍、视力或听力障碍； 4. 合并急性感染，手术创伤，恶性肿瘤者，骨骼肌肉系统畸形，心脑血管疾病，或运动功能障碍不能进行太极柔力球运动； 5. 尚未有效控制的高血压以及低血压患者； 6. 准备接受肾移植患者。

Exclusion criteria：

1. Patients undergoing dialysis using other vascular access; 2. Those who are unable to take care of themselves in daily life; 3. Mental disorders, cognitive impairments, visual or hearing impairments; 4. Those with acute infections, surgical trauma, malignant tumors, skeletal and muscular system malformations, cardiovascular and cerebrovascular diseases, or motor dysfunction who are unable to perform Tai Chi Softball exercises; 5. Patients with hypertension and hypotension that have not been effectively controlled; 6. Patients preparing for kidney transplantation.

干预组在接受常规宣教、橡皮握力圈锻炼的基础上接受太极柔力球运动。在对照组常规护理基础上，患者来院透析日，上机前统一进行太极柔力球运动教学，非透析日进行居家锻炼，目标时间是一周三次、每次运动时间达20-30分钟，循序渐进，根据患者的个体状况分次进行，部分患者也可以由少量多次累计达到20-30分钟。

The intervention group received Tai Chi soft force ball exercise on the basis of regular education and rubber grip strength ring training. On the basis of the routine care of the control group, on the day patients come to the hospital for dialysis, they are uniformly taught Tai Chi softball exercise before using the machine. On non-dialysis days, they exercise at home. The target time is three times a week, with each exercise lasting 20 to 30 minutes. The exercise is carried out gradually, and it is done in batches according to the individual conditions of the patients. Some patients can also accumulate 20 to 30 minutes from small amounts to multiple times.

由责任护士给予患者自体动静脉内瘘侧上肢常规宣教，包括禁止在内瘘侧肢体测血压、输液及长时间压迫和屈曲，不提重物；衣着宽松，每天一听、二摸、三看、四感觉，听局部有无猫喘，摸有无震颤，看有无出血、血肿，感觉有无疼痛，有异常及时就诊；局部红肿、热、痛，有脓性分泌物，提示感染，应及时就诊。选在透析结束24小时后行内瘘侧肢体锻炼，使用常规橡皮握力圈进行锻炼，一天3次，每次10-15分钟。

The responsible nurse should provide the patient with routine education on the upper limb on the side of the autologous arteriovenous fistula, including prohibiting blood pressure measurement, intravenous infusion, prolonged compression and flexion of the limb on the side of the fistula, and not lifting heavy objects. Wear loose clothing. Every day, listen, touch, look and feel. Listen for any cat panting in the local area, touch for any tremors, look for any bleeding or hematoma, and feel for any pain. Seek medical attention promptly if there are any abnormalities. Local redness, swelling, heat, pain, and purulent discharge suggest infection. Medical attention should be sought promptly. The limb on the side of the internal fistula should be exercised 24 hours after the end of dialysis. Use a regular rubber grip strength ring for the exercise, three times a day, for 10 to 15 minutes each time.
