研究目的：

观察老年髋部骨折手术时机对患者临床结局的影响，通过患者术前、术中及术后数据的综合分析，总结影响老年髋部骨折手术结局的主要因素。 基于老年髋部骨折手术患者的术前（合并症、营养状况、基础疾病治疗用药等）、术中（出血、血压波动等）及术后（循环、呼吸、神经、泌尿等系统并发症）全流程数据，综合分析老年髋部骨折患者不同手术时机下手术麻醉管理与患者结局的关联，总结影响患者主要并发症（呼吸系统疾病、谵妄、深静脉血栓等，需要临床干预的并发症）住院死亡率的核心因素。  

Objectives of Study：

To observe the impact of the timing of hip fracture surgery in the elderly on the clinical outcomes of patients, through a comprehensive analysis of the preoperative, intraoperative and postoperative data of the patients, summarize the main factors influencing the surgical outcomes of hip fractures in the elderly. Based on the preoperative (comorbidities, nutritional status, treatment of underlying diseases, etc.), intraoperative (bleeding, blood pressure fluctuations, etc.) and postoperative (circulatory, respiratory, nervous, urinary and other system complications) full-process data of elderly patients undergoing hip fracture surgery, conduct a comprehensive analysis of the association between surgical anesthesia management and patient outcomes under different surgical timing, and summarize the core factors influencing the in-hospital mortality rate of major complications (respiratory system diseases, delirium, deep vein thrombosis, etc., complications that require clinical intervention).

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄>=65岁； 2.行髋部骨折手术（全髋关节置换术、半髋关节置换术、空心钉内固定术、股骨近端防旋髓内钉内固定术）； 3.临床资料完整，可提取术前麻醉评估、术中麻醉管理、术后麻醉相关并发症等核心数据。

Inclusion criteria

1. Age >= 65 years old; 2. Undergoing hip fracture surgery (total hip arthroplasty, hemiarthroplasty, cannulated screw internal fixation, proximal femoral anti-rotation intramedullary nail internal fixation); 3. Complete clinical data available, enabling extraction of core data such as preoperative anesthesia assessment, intraoperative anesthesia management, and postoperative anesthesia-related complications.

排除标准：

1.病理性骨折（如肿瘤转移、骨质疏松性重度病理性骨折）、陈旧性髋部骨折（伤后超过2周）； 2.合并严重凝血功能障碍（如血友病、严重肝病致凝血因子缺乏）、严重感染性疾病（如败血症、感染性休克）、终末期脏器功能衰竭（心、肝、肾、脑功能衰竭）； 3.认知功能完全丧失或无法配合随访评估的患者； 4.中途转院、自动出院或术后30天内失访的患者。

Exclusion criteria：

1. Pathological fractures (such as tumor metastasis, severe pathological fractures due to osteoporosis), old hip fractures (occurring more than 2 weeks after injury); 2. Complicated with severe coagulation dysfunction (such as hemophilia, severe liver disease resulting in deficiency of coagulation factors), severe infectious diseases (such as sepsis, septic shock), end-stage organ failure (heart, liver, kidney, brain failure); 3. Patients with complete loss of cognitive function or unable to cooperate with follow-up assessment; 4. Patients who were transferred to another hospital halfway, automatically discharged, or lost follow-up within 30 days after surgery.
