研究目的：

主要目的：探讨“医院-社区-家庭”三元联动延续护理模式对糖尿病足高危人群的防控效果。 次要目的： 1.评估该模式对患者血糖水平（FBG、2hPBG、HbA1c）的影响； 2.评估患者足部护理知识掌握情况（NAFF 量表）； 3.评估糖尿病足溃疡发生率； 4.评估患者自我管理行为（SDSCA 量表）； 5.评估患者生活质量（DSQL 量表）。  

Objectives of Study：

Primary Objective: To explore the preventive and control effects of the "Hospital-Community-Family" triadic linked continuous care model on high-risk populations for diabetic foot. Secondary Objectives: 1. To evaluate the impact of this model on patients' blood glucose levels (FBG, 2hPBG, HbA1c); 2. To assess patients' mastery of foot care knowledge (NAFF scale); 3. To evaluate the incidence of diabetic foot ulcers; 4. To assess patients' self-management behaviors (SDSCA scale); 5. To evaluate patients' quality of life (DSQL scale).

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.明确诊断为2型糖尿病，并符合上述《糖尿病足基层筛查与防治专家共识》界定的糖尿病足高危人群标准。 2.常住于研究覆盖的社区范围，预计未来1年内居住地稳定，且至少有1名共同居住或能提供日常照护的家属/监护人。 3.年龄≥18岁，意识清楚，具备基本沟通理解能力，自愿参与本研究并签署书面知情同意书。

Inclusion criteria

1. Clearly diagnosed with type 2 diabetes and meeting the criteria for high-risk diabetic foot population as defined in the aforementioned "Expert Consensus on Primary Screening and Prevention of Diabetic Foot." 2. Permanently residing within the community covered by the study, expected to have a stable residence in the next year, and having at least one cohabiting family member or guardian who can provide daily care. 3. Aged ≥18 years, clear consciousness, possessing basic communication and understanding abilities, voluntarily participating in this study and signing a written informed consent form.

排除标准：

1.当前存在活动性糖尿病足溃疡、坏疽或严重感染（Wagner分级≥2级）；
2.合并严重心脑血管疾病、终末期肾病、严重肝功能障碍、恶性肿瘤活动期或严重认知障碍；
3.正在参与其他可能干扰本研究结果的干预性临床试验。

Exclusion criteria：

1.Current active diabetic foot ulcer, gangrene, or severe infection (Wagner grade ≥ 2);
2.Comorbid with severe cardiovascular/cerebrovascular diseases, end-stage renal disease, severe liver dysfunction, active malignant tumor, or severe cognitive impairment;
3.Currently participating in other interventional clinical trials that may interfere with the results of this study.
