研究目的：

1.通过质性访谈，挖掘BO患儿及家长在居家呼吸康复中的核心障碍特征与需求，重点聚焦时间安排冲突及患儿治疗与康复主动性缺乏两大关键问题； 2.基于文献回顾、质性访谈及德尔菲法，构建基于零时间运动理念的青少年BO患者的居家运动方案；并初步验证ZTEx干预方案对BO患儿康复依从性、自我效能感及运动耐力的改善效果，了解患儿及家长对方案的反馈与意见。  

Objectives of Study：

1. Through qualitative interviews, explore the core obstacles and needs of children with BO and their parents in home-based respiratory rehabilitation, focusing on two key issues: time scheduling conflicts and the lack of initiative in the treatment and rehabilitation of the children; 2. Based on literature review, qualitative interviews, and the Delphi method, construct a home-based exercise plan for adolescent children with BO based on the zero-time movement concept; and preliminarily verify the improvement effects of the ZTEx intervention plan on the rehabilitation compliance, self-efficacy, and exercise endurance of children with BO, and understand the feedback and opinions of the children and their parents on the plan.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

BO患儿： 1.依据患儿症状和体征并经高分辨率CT诊断为闭塞性细支气管炎患儿，即符合BO诊断标准； 2.年龄10～18岁； 3.患儿及家长了解本方案研究内容，且同意参加本研究。 照顾者 1.照顾的患儿确诊为BO； 2.照顾的患儿未合并异物吸入、支气管发育不良等其他肺部阻塞性疾病及其他基础疾病，无运动禁忌证，如不稳定性骨折、肺栓塞； 3.患儿照顾者为BO患儿家庭成员； 4.有正常的听说读写和理解能力； 5.自愿参与本次研究，签署知情同意书。

Inclusion criteria

Children with BO: 1. Diagnosed as having obliterative bronchiolitis based on the symptoms and signs of the children and confirmed by high-resolution CT, which meets the diagnostic criteria for BO; 2. Age 10 to 18 years old; 3. The children and their parents understand the research content of this protocol and agree to participate in this study. Caregivers 1. The children under their care have been diagnosed with BO; 2. The children under their care do not have other obstructive pulmonary diseases such as foreign body inhalation, bronchial dysplasia, or other underlying diseases, and have no contraindications for exercise, such as unstable fractures or pulmonary embolism; 3. The caregivers of the BO children are family members of the BO children; 4. Have normal abilities in listening, speaking, reading, writing and understanding; 5. Voluntarily participate in this study and sign the informed consent form.

排除标准：

BO患儿 1.合并其他呼吸道病原体感染、儿童哮喘、各种先天支气管肺发育异常、气道软化等疾病； 2.合并先天性心脏病等引起的喘息、气促、呼吸困难等； 3.存在认知、言语、情感等障碍不能配合本研究的患儿。 照顾者 语言表达不清或沟通障碍。

Exclusion criteria：

Children with bronchopulmonary dysplasia (BO) 1. Complicated with other respiratory pathogens infection, childhood asthma, various congenital bronchopulmonary dysplasia, airway softening and other diseases; 2. Complicated with wheezing, shortness of breath, breathing difficulties caused by congenital heart disease and other conditions; 3. Children with cognitive, speech, emotional and other disorders who cannot cooperate with this study. Caregivers Inability to express themselves clearly or communication disorders.

1.健康教育 （1）认知建立：通过面对面培训，向患儿及家长介绍PIBO疾病特点、呼吸康复的重要性及ZTEx的理念（无需专用时间、场地或设备）。 （2）动作教学：对家长和患儿进行培训，确保理解ZTEx概念并掌握2-3个核心动作。 （3）目标管理：指导患儿及家长设定个性化运动目标，提供简单易用的运动记录表，鼓励家庭记录并分享进展。 2.时间安排 将ZTEx无缝融入PIBO患儿的日常生活，解决学业与康复的时间冲突，培养自主健康行为习惯。 3.依从性管理 （1）信息推送与提醒：主要通过微信平台，定期向家长发送内容生动、形式多样的指导信息，包括融合了卡通元素的图文说明和短视频教程，并进行运动提醒。 （2）沟通与激励机制：在沟通中注重使用鼓励性语言，并对患儿完成训练的努力给予积极反馈。同时，引入“健康积分”等游戏化元素，建立奖励机制，以提升长期参与的趣味性和主动性。 （3）记录工具与资源支持：为患儿提供卡通风格的实体记录手册（如手账本），并辅以线上打卡功能，鼓励通过记录完成情况的方式进行自我监督，形成持续干预的良性循环。 （4）互动分享与监督：创建专属微信群组，在获得伦理同意和家庭自愿的前提下，鼓励家长分享孩子参与运动的照片、视频或心得，通过营造社群氛围来促进相互鼓励与坚持。

(2) Movement Instruction: Train parents and children to ensure they understand the concept of ZTEx and master 2-3 core movements. (3) Goal Management: Guide children and parents to set personalized exercise goals and provide simple and easy-to-use exercise record sheets, encouraging families to record and share progress. 2. Time Arrangement Integrate ZTEx seamlessly into the daily life of PIBO patients, resolve the conflict between studies and rehabilitation, and cultivate autonomous healthy behavior habits. 3. Compliance Management (1) Information Push and Reminders: Mainly through the WeChat platform, send parents content-rich and diverse guidance information regularly, including illustrated text and video tutorials with cartoon elements, and provide exercise reminders. (2) Communication and Incentive Mechanism: In communication, pay attention to using encouraging language and provide positive feedback to the efforts of children in completing training. At the same time, introduce game elements such as "health points" to establish a reward mechanism to enhance the interest and initiative of long-term participation. (3) Record Tools and Resource Support: Provide children with physical record books in cartoon style (such as notebook books), and supplement with online check-in functions, encouraging self-monitoring through the completion of records to form a positive cycle of continuous intervention. (4) Interactive Sharing and Supervision: Create an exclusive WeChat group, with ethical approval and family voluntary participation, encourage parents to share photos, videos or experiences of their children's participation in exercise, and create a community atmosphere to promote mutual encouragement and persistence
