研究目的:

研究并分析上肢机器人联合功能性电刺激对不完全性颈髓损伤上肢功能恢复的疗效影响,探索制定针对不完全性颈髓损伤患者更为高效合理的上肢功能恢复的训练方案。  

Objectives of Study:

To study and analyze the therapeutic effect of upper limb robots combined with functional electrical stimulation on the recovery of upper limb function in patients with incomplete cervical spinal cord injury, and explore the formulation of a more efficient and reasonable training plan for the recovery of upper limb function for patients with incomplete cervical spinal cord injury.

Description for medicine or protocol of treatment in detail:

 

纳入标准:

1.影像学(CT/MRI)和临床检查确诊为脊髓损伤,神经损伤平面在 C1-C6, 损伤程度为 C/D 型。 2.病程 6 个月以内; 3.入组前病情稳定,且可持续轮椅坐位 1 小时以上; 4.年龄为 15-65 岁。

Inclusion criteria

1.The spinal cord injury was confirmed by imaging (CT/MRI) and clinical examination. The nerve injury level was at C1-C6, and the degree of injury was of type C/D. 2.The disease course was within 6 months. 3.Before enrollment, the patient's condition was stable and they could maintain a wheelchair sitting position for more than 1 hour. 4.The age range was 15-65 years old.

排除标准:

1.合并其他严重外伤、严重心血管及内科疾病者; 2.严重并发症:如严重压疮、感染、静脉血栓等需卧床治疗; 3.认知障碍且不能配合者; 4.不服从研究协议者。

Exclusion criteria:

1.Those with other severe injuries, severe cardiovascular and internal medical conditions; 2.Severe complications: such as severe pressure sores, infections, venous thrombosis, etc., requiring bed rest treatment; 3.Those with cognitive impairments and unable to cooperate; 4.Those who do not comply with the research protocol.

受试者每日接受 20min/侧常规上肢功能训练,包括阶梯插板训练、 阶梯套圈训练、上肢推举训练、体操棒负重抬举训练等。训练频率为 6 日/周, 连续 4 周。

受试者每日接受 20分钟/侧上肢机器人联合 功能性电刺激训练; 功能性电刺激采用功能性电刺激训练仪(FUL4),电流强度为 8-50毫安,脉宽 250μs,脉 冲频率 20-70HZ,刺激肌群:三角肌前束和肱三头肌长头。训练频率为 6 日/周, 连续 4 周。

Intervention:

The subjects received 20 minutes of robot-assisted combined FES training for each arm every day; The FES was conducted using a functional electrical stimulation training device (FUL4), with current intensity ranging from 8 to 50 mA, pulse width of 250 μs, pulse frequency of 20 to 70 Hz, and the stimulated muscle groups being the anterior deltoid and the long head of the triceps brachii. The training frequency was 6 days per week, for a total of 4 weeks.

独立于实验的研究者刘莹莹女士,采用Excel随机函数生成的随机数字进行简单随机化,将受试者按1:1:1比例随机分配至常规组、上肢机器人组、上肢机器人结合FES组。

Researcher Liu Yingying, who is independent of the experiment, used random numbers generated by Excel's random function for simple randomization. She randomly assigned the subjects to the conventional group, the upper limb robot group, and the upper limb robot combined with FES group in a 1:1:1 ratio.
