研究目的：

本研究旨在探讨健侧运动皮层高频 rTMS对严重脑卒中上肢运动功能恢复的疗效，并以fNIRS评估其对大脑皮层可塑性的影响，为脑卒中后严重上肢运动障碍的患者提供新康复策略及理论基础。  

Objectives of Study：

This study aims to investigate the efficacy of high-frequency rTMS on the motor cortex of the unaffected side in the recovery of upper limb motor function in severe stroke, and to assess its impact on cortical plasticity using fNIRS, providing new rehabilitation strategies and theoretical basis for patients with severe upper limb motor dysfunction after stroke.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①脑卒中诊断符合中华医学会神经病学分会制定的中国脑出血或急性缺血性脑卒中诊治指南，且经过颅脑CT或颅脑MRI等影像学检查证实；②初次、单侧发病，病程2周至3个月；③年龄30 -85岁；④存在严重单上肢运动障碍，Brunnstrom ＜3期，且Fugl-Meyer 上肢运动功能评分<22/66；⑤本人或代理人提供书面知情同意。

Inclusion criteria

1. Stroke diagnosis consistent with the diagnostic and treatment guidelines for intracerebral hemorrhage or acute ischemic stroke in China established by the Neurology Branch of the Chinese Medical Association, and confirmed by imaging examinations such as cranial CT or cranial MRI; 2. First-time, unilateral onset, with disease duration ranging from 2 weeks to 3 months; 3. Age between 30 and 85 years; 4. Presence of severe unilateral upper limb motor dysfunction, Brunnstrom stage < 3, and Fugl-Meyer Upper Extremity Motor Function Score < 22/66; 5. Written informed consent provided by the participant or their legal representative.

排除标准：

①有癫痫发作史；②全身严重损伤或者伴有其他严重疾病；③严重的失语或认知障碍，不能配合完成本研究；④存在经颅磁刺激的任何禁忌症（例如脑内金属植入物或心脏起搏器）；⑤妊娠期妇女。

Exclusion criteria：

1. History of epileptic seizures; 2. Severe systemic injury or accompanied by other serious illnesses; 3. Severe aphasia or cognitive impairment, unable to cooperate and complete the study; 4. Presence of any contraindications for transcranial magnetic stimulation (such as intracranial metal implants or cardiac pacemakers); 5. Pregnant women.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

研究者根据受试者的原始观察记录，将数据及时、完整、正确、清晰地载入病例报告表，录入采用相应的数据库系统双人双机录入，之后对数据库进行两遍比对。电子数据文件分类保存，并有多个备份保存于不同磁盘或者记录介质上，妥善保存，防止损坏。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

Based on the original observation records, researchers will completely write accurate data into case report forms in time. Input the data into corresponding database system by two special researchers with two computers respectively. After that, researchers compare two database twice and electronic data will be conserved and backup.
