研究目的：

1、通过此研究，证实Ⅰ期顺逆行双镜联合手术在复杂性上尿路结石治疗中的优势。 2、为上尿路复杂性结石治疗验证一种安全、有效的治疗方式，最终目标是提高本区域内泌尿系上尿路复杂结石I期清除率，减少患者住院次数，缩短住院时间，减轻患者经济负担。  

Objectives of Study：

1.Through this study, to demonstrate the superiority of one-stage anterograde-retrograde dual endoscopic surgery in the treatment of complex upper urinary tract stones. 2.To validate a safe and effective treatment modality for complex upper urinary tract stones, with the ultimate goals of improving the single-session stone clearance rate within our region, reducing the number of hospital admissions, shortening the length of hospital stay, and alleviating the economic burden on patients.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

铸型肾结石（结石位于肾盂，分支进入肾盏）、多发性肾结石（≥２枚结石）、巨大肾结石（直径＞2.5cm），经Ｂ超、ＫＵＢ＋ＩＶＰ、ＣＴ三维成像、尿常规等检查，诊断明确，术前检查完善，患者知情同意，具有明确的手术适应证，无相关手术禁忌证。

Inclusion criteria

Patients with staghorn calculi (stones located in the renal pelvis with branches extending into the calyces), multiple renal calculi (≥2 stones), or large renal calculi (diameter >2.5cm), who have a confirmed diagnosis through examinations such as B-ultrasound, KUB + IVP, three-dimensional CT imaging, and urinalysis, with complete preoperative assessments, informed consent, clear surgical indications, and no relevant surgical contraindications.

排除标准：

合并心、脑、肺、肝、肾等重要脏器功能障碍，难以耐受手术；原发性高血压、糖尿病等疾病未控制；未控制的急性尿路感染、肾积脓、结核活动期；游走肾、移植肾、重度肾下垂、肾盂输尿管连接部狭窄。

Exclusion criteria：

Patients with dysfunction of vital organs (such as cardiac, cerebral, pulmonary, hepatic, or renal) that precludes tolerance for surgery; uncontrolled primary conditions like hypertension or diabetes; uncontrolled acute urinary tract infection, pyonephrosis, or active tuberculosis; or those with conditions including mobile kidney, transplanted kidney, severe nephroptosis, or ureteropelvic junction obstruction.

本研究拟对安宁市医共体泌尿外科一段时间内（2026.1.1-2026.12.30）诊断为复杂上尿路结石并且需行PCNL手术的患者，根据纳入及排除标准，资料收集者采用在线随机化工具(https://online-random.com/cn/)将研究患者按1:1的比例分配到顺行经皮肾镜联合逆行软镜组或单通道经皮肾镜组产生随机序列。

Randomization Procedure (please state who generates the random number sequence and by what method)：

This study plans to enroll patients diagnosed with complex upper urinary tract stones within the Anning City Medical Consortium's Urology Department during a specified period (January 1, 2026, to December 30, 2026) and scheduled for PCNL. Based on the inclusion and exclusion criteria, the data collector will use an online randomization tool (https://online-random.com/cn/) to allocate enrolled patients in a 1:1 ratio to either the combined antegrade percutaneous nephrolithotomy with retrograde flexible ureteroscopy group or the single-tract percutaneous nephrolithotomy group, thereby generating the random sequence.

盲法：

本研究对患者和手术医师不设盲。主要结局（术后CT结石清除率）将由对分组信息不知情的独立影像学专家评估。数据录入与统计分析阶段也将对分组保持盲态。

Blinding：

Neither the patients nor the operating surgeons were blinded in this study. The primary outcome (postoperative CT single-session stone-free rate) will be assessed by an independent radiologist who is blinded to group allocation. Blinding will also be maintained for personnel involved in data entry and statistical analysis.
