研究目的：

评估颈部动态 MRI 指导下的个体化手术诊疗方案对脊髓型颈椎病患者术后神经功能恢复（以日本骨科协会 JOA 评分为核心评价指标）的临床疗效。  

Objectives of Study：

To evaluate the clinical effect of individualized surgical diagnosis and treatment under the guidance of neck dynamic MRI on postoperative neurological recovery (Japanese Orthopaedic Association JOA score as the core evaluation index) in patients with cervical spondylotic myelopathy.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1、符合脊髓型颈椎病诊断标准； 2、年龄 18-60 周岁，性别不限，生命体征平稳者； 3、病程＞3 个月； 4、自愿接受对应治疗方案（观察组术前行动态 MRI，对照组术前常规 MRI）； 5、治疗前 4 周无糖皮质激素用药史； 6、签署知情同意书。

Inclusion criteria

1. Meet the diagnostic criteria of cervical spondylotic myelopathy; 2. Age of 18-60 years old, both sexes, stable vital signs; 3. Course of disease > 3 months; 4. Voluntarily accept the corresponding treatment plan (the observation group underwent dynamic MRI before surgery, and the control group underwent conventional MRI before surgery); 5. No history of glucocorticoid use 4 weeks before treatment; 6. Signed the informed consent form.

排除标准：

1.病历中未明确符合脊髓型颈椎病诊断标准，或基线资料（病程、影像学报告、症状记录）不完整，无法完成疗效与安全性评价者； 2.病历记录显示近 3 个月参与过其他临床研究，存在干预交叉或数据干扰可能者； 3.既往有严重颈椎创伤史、颈椎手术史或其他脊柱相关手术史（以手术记录、住院病史记载为准），影响手术方案可比性及疗效评估者； 4.病历中明确记录合并心、肝、肾等重要脏器功能严重损伤、造血功能异常或恶性肿瘤等严重基础疾病，可能增加研究偏倚或影响随访结局者； 5.有阿片类药物、镇静催眠药依赖史或长期酒精滥用史（以病历用药记录、病史自述为准），可能干扰神经功能评估者； 6.病历记录为孕妇、哺乳期女性（基于产科病史、检查记录判定）； 7.治疗前 4 周病历中有糖皮质激素使用记录，可能影响神经功能或炎症反应评估，干扰疗效判定者；

Exclusion criteria：

1. Patients who did not meet the diagnostic criteria of cervical spondylotic myelopathy in the medical record, or whose baseline data (course of disease, imaging report, symptom record) were incomplete, and the efficacy and safety evaluation could not be completed; 2. Medical records showed that they had participated in other clinical studies in the past 3 months, with intervention crossover or data interference possible; 3. patients with a history of severe cervical spine trauma, cervical spine surgery or other spine-related surgery (based on operation records and hospitalization history), which may affect the comparability of surgical programs and efficacy evaluation; 4. patients with serious underlying diseases such as heart, liver, kidney or other important organ dysfunction, hematopoietic dysfunction or malignant tumors, which may increase the study bias or affect the follow-up outcome; 5. patients with a history of dependence on opioids, sedatives and hypnotics or long-term alcohol abuse (based on the medication record or self-history), which may interfere with the assessment of neurological function; 6. pregnant or lactating women (based on obstetric history and examination records); 7. The use of glucocorticoids in the medical records for 4 weeks before treatment, which may affect the evaluation of neurological function or inflammatory response and interfere with the evaluation of efficacy;

This study was a retrospective cohort study on the planning and implementation of individualized surgical diagnosis and treatment plan for cervical spondylotic myelopathy based on cervical dynamic MRI. Data collection and management strictly followed the clinical research standards. The standardized design of Case Record Form (CRF) and the digital management of Electronic Data Capture and management system (EDC) were combined to ensure the integrity, accuracy and traceability of research data
