研究目的：

本研究旨在构建一个整合 mpMRI、微结构成像、分子成像技术及生成式图像技术的创新评估体系，系统性地解决前列腺癌在诊断、分期和预后预测中的核心临床挑战。  

Objectives of Study：

This study aims to develop an integrated assessment system incorporating mpMRI, MR cytometry, molecular imaging, and generative imaging technologies to systematically address the key clinical challenges in the diagnosis, staging, and prognosis prediction of prostate cancer.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

健康志愿者： 1. 既往无前列腺相关疾病及治疗史 2. 年龄>=18 岁 3. MRI 扫描前签署知情同意书 临床怀疑前列腺肿瘤患者： 1. 临床怀疑前列腺癌而行进行完整前列腺常规MRI、微结构成像（TDD、IVIM、DKI、MD-MRI）及分子代谢成像技术（APT、超极化MRI）扫描 2. MRI 扫描后 3 个月内经穿刺活检证实为前列腺癌 3. MRI 扫描后 3 个月内于本院行根治性前列腺切除术

Inclusion criteria

Healthy Volunteers: 1. No prior history of prostate-related diseases or treatments; 2. Aged >= 18 years; 3. Signed informed consent prior to MRI examination. Patients with Suspected Prostate Tumor: 1. Underwent complete prostate conventional MRI, MR cytometry (TDD, IVIM, DKI, MD-MRI), and molecular metabolic imaging (APT, hyperpolarized MRI) due to clinical suspicion of prostate cancer; 2. Pathologically confirmed prostate cancer by biopsy within 3 months after MRI examination; 3. Underwent radical prostatectomy at our institution within 3 months after MRI examination.

排除标准：

健康志愿者： 1.因身体或心理原因无法完成多次 MRI 扫描的患者 2.MRI 扫描后提示存在前列腺相关疾病（如前列腺重度增生、前列腺炎等） 临床怀疑前列腺肿瘤患者： 1.MRI 上病灶不可见（PI-RADS＜3 分） 2.既往有前列腺疾病相关治疗史

Exclusion criteria：

Healthy Volunteers: 1. Unable to complete multiple MRI scans due to physical or psychological reasons; 2. MRI findings suggestive of prostate-related diseases (e.g., severe benign prostatic hyperplasia, prostatitis, etc.). Patients with Suspected Prostate Tumor: 1. Lesions not visible on MRI (PI-RADS < 3); 2. Prior history of prostate-related treatments.

Diagnostic Tests：

金标准或参考标准（即可准确诊断某疾病的单项方法或多项联合方法，在本研究中用于诊断是否有该病的临床参考标准）：

1.依据2019版国际泌尿病理学会指南（ISUP）对穿刺或根治性切除组织进行Gleason评分； 2.包膜外侵犯判定： 经组织病理学检查证实的前列腺包膜外肿瘤浸润； 3.手术切缘阳性判定： 经组织病理学检查证实的手术切缘肿瘤残留； 4.生化复发判定：术后血清总PSA连续两次 >= 0.2ng/ml或复查中PSA连续上升至>= 0.1ng/ml。

Gold Standard or Reference Standard (The clinical reference standards required to establish the presence or absence of the target condition in the tested population in present study):

1. Criteria for Gleason score: Gleason score was performed according to the 2019 International Society of Urological Pathology (ISUP) guidelines on biopsy or radical prostatectomy specimens. 2. Criteria for Extraprostatic extension: extraprostatic extension was defined as histopathologically confirmed tumor infiltration beyond the prostatic capsule. 3. Criteria for Positive surgical margin: positive surgical margin was defined as histopathologically confirmed tumor presence at the surgical resection margin. 4. Criteria for Biochemical Recurrence: biochemical recurrence was defined as two consecutive postoperative serum total PSA levels >= 0.2 ng/mL, or a continuous rise in PSA to >= 0.1 ng/mL during follow-up.
