Objectives of Study: |
Based on multi-center samples, an intelligent diagnostic model for WMH was constructed. The clinical diagnostic efficacy of this model was systematically evaluated to provide early warning for vascular mild cognitive impairment, enable early intervention, and delay the progression to dementia. |

|
纳入标准: |
1.年龄50~80岁高血压病患者; ? 2.头颅MRI提示缺血性WMH; ? 3.无卒中、头部外伤、脑炎、脑积水、癫痫、帕金森病、阿尔茨海默症、额颞叶痴呆、路易体痴呆等中枢神经系统疾病史, 无引起认知障碍的全身系统性疾病;? 4.无先天精神发育迟缓或焦虑抑郁症等精神疾病史; 5.未服用影响认知功能的药物;? 6.可以配合完成认知功能评价[简易精神状态量表(minimentalstate examination, MMSE)评分, 文盲>13分,小学文化程度>19分, 初中及以上文化程度>24分]; 7.签署知情同意书。 |

Inclusion criteria |
1. Hypertensive patients aged 50 to 80 years. 2. Presence of ischemic white matter hyperintensities (WMH) on cranial magnetic resonance imaging (MRI). 3. No history of central nervous system diseases such as stroke, head trauma, encephalitis, hydrocephalus, epilepsy, Parkinson's disease, Alzheimer's disease, frontotemporal dementia, or Lewy body dementia, and no systemic diseases known to cause cognitive impairment. 4. No history of congenital mental retardation or psychiatric disorders such as anxiety or depression. 5. Not taking any medications that affect cognitive function. 6. Ability to cooperate with completing cognitive assessments [a Mini-Mental State Examination (MMSE) score of >13 for illiterate individuals, >19 for those with primary school education, and >24 for those with a junior high school education or above]. 7. Provision of signed informed consent. |

排除标准: |
1.非血管源性WMH,如多发性硬化、结节病、脑部放疗、脑白质营养不良等; 2.无皮质和(<或)皮质下的非腔隙性梗死、脑出血; 3.头颅MRI明确提示皮层或海马萎缩; 4.其他变性疾病引起的认知障碍:如阿尔茨海默病、 路易体痴呆、额颞叶痴呆、帕金森病等; 5.其他脑部病变:肿瘤、脑积水、外伤、梅毒、获得性免疫缺陷综合征、克雅氏病等; 6.严重精神疾病及癫痫、酒精及药物滥用、中毒和代谢异常等; 7.头颅MRI检查未见异常或无法配合完成MRI检查者。 |

Exclusion criteria: |
1. Presence of WMH from non-vascular origins, such as multiple sclerosis, sarcoidosis, cerebral radiation therapy, or leukodystrophies. 2. Presence of non-lacunar infarcts (cortical and/or subcortical) or cerebral hemorrhage. 3. Cranial MRI clearly indicating cortical or hippocampal atrophy. 4. Cognitive impairment attributable to other neurodegenerative diseases, such as Alzheimer's disease, Lewy body dementia, frontotemporal dementia, or Parkinson's disease. 5. Other cerebral pathologies, including tumors, hydrocephalus, trauma, neurosyphilis, acquired immunodeficiency syndrome (AIDS), or Creutzfeldt-Jakob disease. 6. Severe psychiatric disorders, epilepsy, alcohol or substance abuse, poisoning, or metabolic abnormalities. 7. Individuals with no abnormalities detected on cranial MRI or those unable to undergo or complete the MRI examination. |

数据采集和管理(说明:数据采集和管理由两部分组成,一为病例记录表(Case Record Form, CRF),二为电子采集和管理系统(Electronic Data Capture, EDC),如ResMan即为一种基于互联网的EDC: |
采集样本人群的基线信息, 包括年龄、性别、教育年限、吸烟饮酒史、糖化血红蛋白、血脂、高血压病程等。影像数据采集:所有研究对象采用固定统一的3.0T磁共振扫描仪进行影像学检查, 扫描序列包括T1WI、T2WI、FLAIR序列,采用统一的扫描参数,所有研究对象均由影像科专业人员进行头颅核磁数据采集。双人协同操作,将获取的样本MRI图像信息输入WMH自动识别系统进行WMH体积的自动识别测算,得到所有样本的WMH体积数据值,并做好记录,结果导入Excel文档中,包括认知评估量表评分。 |

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture: |
Baseline information of the study cohort was collected, including age, sex, years of education, history of smoking and alcohol consumption, glycated hemoglobin (HbA1c), lipid profiles, and duration of hypertension.Imaging data acquisition: All participants underwent neuroimaging examinations using a fixed and uniform 3.0T MRI scanner. The scanning protocols included T1-weighted imaging (T1WI), T2-weighted imaging (T2WI), and fluid-attenuated inversion recovery (FLAIR) sequences, employing consistent scanning parameters. All cranial MRI data were acquired by professional radiologists.Two operators worked collaboratively to input the obtained MRI images into the automatic WMH identification system for automated WMH volume quantification. The WMH volume data for all samples were obtained, systematically recorded, and the results—including cognitive assessment scale scores—were exported into an Excel document. |
