研究目的：

1.比较不同生活方式减重措施的减重效果。 2.评估不同生活方式减重方法对糖尿病前期和/或 2 型糖尿病患者骨密度的影响。 3.探寻不同生活方式减重方法对糖尿病前期和/或 2 型糖尿病患者骨形成、骨吸收机制的影响。  

Objectives of Study：

1. Compare the weight loss effects of different lifestyle weight loss measures. 2. Evaluate the impact of different lifestyle weight loss methods on bone density in patients with prediabetes and/or type 2 diabetes. 3. Explore the influence of different lifestyle weight loss methods on bone formation and bone resorption mechanisms in patients with prediabetes and/or type 2 diabetes.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.糖尿病前期患者：根据糖尿病前期的诊断标准，纳入空腹血糖受损、糖耐量减低的患者 2.2型糖尿病患者：根据2型糖尿病诊断标准，纳入空腹血糖>=7.0mmol/L、随机血糖>=11.1mmol/L或口服葡萄糖耐量试验（OGTT）2小时血糖>=11.1mmol/L的患者 3.根据肥胖诊断标准：身体质量指数BMI>=28kg/m2，或腰围符合肥胖相关标准，男性腰围>=90cm，女性腰围>=85cm 4.年龄范围：18-65周岁（含18周岁和65周岁），性别不限 5.近3个月内血糖控制相对稳定，降糖方案相对稳定，糖化血红蛋白HbA1c<=8.5%

Inclusion criteria

1. Patients with prediabetes: According to the diagnostic criteria for prediabetes, include patients with impaired fasting glucose or impaired glucose tolerance. 2. Patients with type 2 diabetes: According to the diagnostic criteria for type 2 diabetes, include patients with fasting blood glucose >= 7.0 mmol/L, random blood glucose >= 11.1 mmol/L, or 2-hour blood glucose in an oral glucose tolerance test (OGTT) >= 11.1 mmol/L. 3. According to obesity diagnostic criteria: Body Mass Index (BMI) >= 28 kg/m2, or waist circumference meeting obesity-related standards: male waist circumference >= 90 cm, female waist circumference >= 85 cm. 4. Age range: 18-65 years old (including 18 and 65), no gender restriction. 5. Blood glucose control relatively stable in the past 3 months, hypoglycemic treatment regimen relatively stable, and glycated hemoglobin HbA1c <= 8.5%.

排除标准：

1.合并酮症酸中毒、高渗高血糖综合征等急性并发症和严重慢性并发症的患者 2.重度骨质疏松患者（T<=-3.5或T<=-2.5+脆性骨折） 3.合并严重心肺功能不全、肝肾功能不全等疾病 4.影响骨代谢的疾病：如甲状旁腺功能亢进或减退、甲状腺功能亢进或减退、库欣综合征、性腺功能减退症、肢端肥大症、骨软化症、成骨不全、乳糜泻、炎症性肠病、慢性肝病、恶性肿瘤等；患有类风湿关节炎、系统性红斑狼疮等自身免疫性疾病 5.近1年内使用过影响骨代谢的药物：如双膦酸盐类、降钙素、甲状旁腺激素类似物、糖皮质激素等 6.近1年内接受过减重手术或计划在研究期间进行减重手术 7.妊娠或哺乳期女性：处于妊娠或哺乳期的女性，以及计划在研究期间妊娠的女性 8.精神心理疾病：患有严重精神疾病、严重认知功能障碍，无法配合研究过程

Exclusion criteria：

1. Patients with acute complications such as ketoacidosis and hyperosmolar hyperglycemic syndrome, as well as severe chronic complications 2. Patients with severe osteoporosis (T <=-3.5 or T <=-2.5 + fragility fractures) 3. Patients with severe cardiopulmonary dysfunction, liver and kidney dysfunction, etc. 4. Diseases affecting bone metabolism: such as hyperparathyroidism or hypoparathyroidism, hyperthyroidism or hypothyroidism, Cushing's syndrome, hypogonadism, acromegaly, osteomalacia, osteogenesis imperfecta, celiac disease, inflammatory bowel disease, chronic liver disease, malignant tumors, etc.; patients with autoimmune diseases such as rheumatoid arthritis, systemic lupus erythematosus, etc. 5. Patients who have used drugs affecting bone metabolism within the past 1 year: such as bisphosphonates, calcitonin, parathyroid hormone analogues, glucocorticoids, etc. 6. Patients who have undergone bariatric surgery in the past 1 year or plan to undergo bariatric surgery during the study period 7. Pregnant or lactating women: Women who are pregnant or lactating, and those who plan to become pregnant during the study period 8. Mental and psychological disorders: Patients with severe mental illness, severe cognitive dysfunction, and those who cannot cooperate with the study process

糖尿病前期或2型糖尿病伴超重或肥胖的患者80例：线上给予饮食方面的指导，避免摄入过多或过少的热量；患者根据自身情况资源选择中等强度有氧运动或抗阻运动，有氧运动每周4-6次，每次累计30分钟以上；抗阻运动每周2-3次，每次累计20-40分钟，每2次运动至少间隔48小时，并加入保护骨密度的干预：摄入钙和维生素D，干预时间为半年，随访时间为3个月、6个月

Intervention：

80 overweight or obese patients with prediabetes or type 2 diabetes received online dietary counseling for appropriate calorie intake and voluntarily selected moderate-intensity aerobic exercise or resistance training based on their conditions. Aerobic exercise was performed 4–6 times weekly for over 30 minutes per session, and resistance training 2–3 times weekly for 20–40 minutes per session with at least 48 hours between sessions. Calcium and vitamin D intake was also advised to preserve bone mineral density. The intervention lasted 6 months, with follow-ups at 3 and 6 months.
