研究目的：

本研究旨在通过随机对照试验，评估经皮耳迷走神经刺激对社区老年人自主神经功能及内在能力的影响，分析其对IC整体水平及多维度（运动、活力、认知、心理、感觉）的改善效果；进一步探讨心率变异性在经皮耳迷走神经刺激促进内在能力改善过程中的中介效应，验证自主神经系统调节作为潜在干预机制的可行性，并评估该干预在社区老年人群中的有效性、依从性及可操作性。  

Objectives of Study：

This study aims to evaluate, through a randomized controlled trial, the effects of transcutaneous auricular vagus nerve stimulation (taVNS) on autonomic function and intrinsic capacity in community-dwelling older adults, and to analyze its impact on overall intrinsic capacity (IC) and improvements across multiple dimensions (Locomotion, vitality, cognition, psychology, and sensory); Furthermore, the study seeks to explore the mediating role of heart rate variability in the process by which taVNS promotes improvements in intrinsic capabilities, to validate the feasibility of autonomic nervous system regulation as a potential intervention mechanism, and to assess the efficacy, adherence, and feasibility of this intervention among community-dwelling older adults.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄>=60周岁； 2.具有基本沟通能力，能够完成问卷及体测； 3.HRV检测可获得至少5分钟连续的心电信号； 4.经过IC筛查，存在IC下降（>=1个域受损为IC下降，即IC总评分>=1分）的社区老年人； 5.签署知情同意书，自愿参与本研究者。

Inclusion criteria

1. Age >= 60 years old; 2. Have basic communication abilities and be able to complete the questionnaire and physical test; 3. HRV testing can obtain at least 5 minutes of continuous ECG signal; 4. Community elders who have IC decline after IC screening (IC decline is defined as impairment in >=1 domain, that is, total IC score >=1 point); 5. Signed informed consent and voluntarily participate in this study.

排除标准：

1.已植入心脏起搏器或除颤器等电子装置； 2.存在严重心血管疾病、呼吸、肝/肾功能不全或其他由医生评估认为不宜接受电刺激者； 3.存在严重精神障碍、重度认知障碍者； 4.存在严重躯体疾病或近期急性发作者； 5.外耳道或耳甲腔存在感染、皮肤破溃或其他无法佩戴设备的情况； 6.目前正在参加另一项行为或药物试验者。

Exclusion criteria：

1.Patients with implanted electronic devices such as pacemakers or defibrillators; 2.Patients with severe cardiovascular disease, respiratory disorders, liver or kidney dysfunction, or other conditions deemed unsuitable for electrical stimulation by a physician; 3.Individuals with severe mental disorders or severe cognitive impairment; 4.Individuals with severe physical illnesses or those who have recently experienced an acute exacerbation; 5.Individuals with infections, skin ulcers, or other conditions in the external auditory canal or concha that prevent the device from being worn; 6.Individuals currently participating in another behavioral or drug trial.

干预组接受经皮耳迷走神经刺激（taVNS）。刺激部位为左侧耳甲腔，刺激参数设置为：频率25 Hz，脉宽250 μs，刺激模式为30 s开/30 s关，刺激强度根据受试者耐受程度调节（以不引起明显不适为宜），每次20分钟，每周5次，共干预2周。

The intervention group received transcutaneous auricular vagus nerve stimulation (taVNS). The stimulation site was the left concha of the ear, with the following parameters: frequency 25 Hz, pulse width 250 μs, and a stimulation pattern of 30 seconds on and 30 seconds off. Stimulation intensity was adjusted according to the participant’s tolerance (set at a level that did not cause significant discomfort). Each session lasted 20 minutes, five times a week, for a total of two weeks.
