基于大型DXA检测数据库,系统分析东北地区人群多个部位骨密度随年龄增长的变化趋势和变化速率,结合人口学基线特征总结骨密度的规律性变化,为制定符合中国人群特征的骨质疏松诊断标准提供科学依据。通过本土化的数据支撑,提高临床骨质疏松诊断的准确性,实现精准的早期筛查和防控。 同时,在总体分析基础上,按是否存在可能显著影响骨密度的特异性疾病(如长期系统性糖皮质激素应用、内分泌疾病、慢性肾脏疾病、风湿免疫性疾病等)及相关用药进行分层或敏感性分析,量化不同疾病背景下年龄相关骨量变化及参考值的差异。 |

Objectives of Study: |
Based on a large-scale DXA (dual-energy X-ray absorptiometry) testing database, this study systematically analyzes the changing trends and rates of bone mineral density (BMD) at multiple skeletal sites with increasing age in the population of Northeast China. By integrating demographic baseline characteristics, it summarizes the regular patterns of BMD variation, aiming to provide a scientific basis for establishing osteoporosis diagnostic criteria tailored to the characteristics of the Chinese population. Supported by localized data, this research seeks to enhance the accuracy of clinical osteoporosis diagnosis and achieve precise early screening and prevention. Furthermore, building on the overall analysis, stratified or sensitivity analyses will be conducted based on the presence of specific diseases that may significantly affect bone mineral density—such as long-term systemic glucocorticoid use, endocrine disorders, chronic kidney disease, rheumatic and autoimmune diseases—as well as related medications. This approach will quantify the differences in age-related bone mass changes and reference values across various disease backgrounds. |

Inclusion criteria |
This study is a single-center retrospective investigation. The study subjects include all patients who underwent DXA bone mineral density measurement in the Rehabilitation Department of Shengjing Hospital Affiliated to China Medical University between November 2016 and December 2026. Data were sourced from bone density test reports and patients' routine clinical records, including information on gender, age, height, weight, medical history, medication use, clinical diagnoses, and laboratory test results. |

为了尽可能利用大数据进行流行病学分析,本研究原则上不设严格的排除标准,不区分原发性和继发性骨质疏松症患者,纳入各临床科室出于不同目的在康复科进行骨密度检查的患者。 |

Exclusion criteria: |
To maximize the utilization of big data for epidemiological analysis, this study does not establish strict exclusion criteria in principle. It does not distinguish between patients with primary or secondary osteoporosis and includes all patients referred from various clinical departments for bone density testing in the Rehabilitation Department for different purposes. |

数据采集和管理(说明:数据采集和管理由两部分组成,一为病例记录表(Case Record Form, CRF),二为电子采集和管理系统(Electronic Data Capture, EDC),如ResMan即为一种基于互联网的EDC: |
CRF:研究团队根据回顾性数据特点制定统一CRF/数据字典,用于从盛京医院康复科DXA检查相关资料中抽取变量。 EDC:采用受控电子数据管理环境对研究数据进行采集、存储与版本管理。医院端导出数据前完成去标识化:Excel数据对患者重新编码生成研究ID(Study ID),删除姓名、证件号等个人标识信息后导出;DXA影像DICOM数据采用Python删除或重编码含个人信息的metadata字段。脱敏后的Excel与DICOM数据导入研究EDC(加密存储、权限控制),仅限经授权研究人员访问,并保留数据导入、清洗与变更的记录以确保数据可追溯。 |

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture: |
CRF: A standardized CRF/data dictionary will be developed to retrospectively extract variables from DXA examinations performed in the Department of Rehabilitation Medicine at Shengjing Hospital of China Medical University. EDC: A controlled electronic data management environment will be used for data capture, storage, and version control. De-identification will be completed prior to data export at the hospital site. For spreadsheet-based data (e.g., Excel exports), patients will be re-coded with a study identifier (Study ID), and direct identifiers (e.g., name, national ID number) will be removed before export. For DXA DICOM data, Python scripts will be used to remove or re-code DICOM metadata fields containing personal identifiers. De-identified Excel and DICOM datasets will then be imported into the study EDC with encrypted storage and access control, accessible only to authorized study personnel. Data import, cleaning, and any modifications will be logged to ensure traceability. |
