研究目的:

主要终点 (Primary Endpoint); 构建基于声学组学特征的 DGE(B 级/C 级)超早期预警模型(AUC > 0.85)。 次要终点 (Secondary Endpoints): 探索 POPF(B 级/C 级) 发生前的特征性声学前驱表现。 分析声学特征与 Cajal 间质细胞(ICC)网络完整性及外周血炎症因子(IL-6, ACh)的相关性。  

Objectives of Study:

Primary Endpoint: Development of an acoustic radiomics–based ultra-early prediction model for clinically relevant delayed gastric emptying (DGE, Grade B/C), with an AUC > 0.85. Secondary Endpoints: 1.Identification of characteristic acoustic precursor signatures preceding clinically relevant postoperative pancreatic fistula (POPF, Grade B/C). 2.Evaluation of the associations between acoustic features, the integrity of the interstitial cells of Cajal (ICC) network, and peripheral inflammatory markers (IL-6 and ACh).

纳入标准:

1.拟在华西医院行择期胰十二指肠切除术(PD)者; 2.年龄 18-80 岁; 3.术前签署知情同意书。

Inclusion criteria

1. Patients scheduled to undergo elective pancreaticoduodenectomy (PD) at West China Hospital. 2. Age between 18 and 80 years. 3. Provision of written informed consent prior to surgery.

排除标准:

1.腹部皮肤破损严重无法佩戴设备者; 2.既往有腹部放疗史者(可能损伤肠神经); 3.术前已存在严重胃肠动力障碍疾病者。

Exclusion criteria:

1. Patients with severe abdominal skin damage that precludes the use of the monitoring device. 2. History of prior abdominal radiotherapy (which may impair enteric neural function). 3. Presence of severe gastrointestinal motility disorders prior to surgery.

Within 6 months after publication of the primary study results, de-identified data (including acoustic radiomics feature data and relevant clinical data) will be made available to qualified researchers upon reasonable request. Applicants are required to submit a research proposal, and access to the data will be granted upon approval by the principal investigator.

数据采集和管理(说明:数据采集和管理由两部分组成,一为病例记录表(Case Record Form, CRF),二为电子采集和管理系统(Electronic Data Capture, EDC),如ResMan即为一种基于互联网的EDC:

本研究采用标准化病例报告表(CRF)和电子数据采集系统(EDC)进行数据管理。临床数据由研究人员录入CRF,并经双人核对确保数据准确性。声学数据由可穿戴设备自动采集并以原始音频格式(.wav)存储,经信号处理后提取声学组学特征(.csv),统一导入EDC系统。 所有数据均进行去标识化处理,并通过唯一受试者编码进行管理。EDC系统(ResMan,http://www.medresman.org/)具备权限控制、数据审计追踪及备份功能,以保障数据安全性与完整性。研究期间定期开展数据核查与质量控制。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture:

Data will be collected and managed using standardized Case Report Forms (CRFs) and an Electronic Data Capture (EDC) system. Clinical data will be entered into the CRFs by study personnel and verified by double-checking to ensure accuracy. Acoustic data will be automatically collected using a wearable device and stored as raw audio files (.wav). Following signal processing, acoustic radiomics features (.csv) will be extracted and uploaded into the EDC system. All data will be de-identified and managed using unique subject identification codes. The EDC system (ResMan, [http://www.medresman.org/](http://www.medresman.org/)) provides role-based access control, audit trails, and data backup functions to ensure data security and integrity. Regular data monitoring and quality control procedures will be conducted throughout the study.
