研究目的：

本项目将构建基于动态患者画像的口腔癌术后居家患者精准症状管理模式并应用，据此形成以下三个研究目标： （1）构建基于多维度信息融合的口腔癌术后居家患者动态画像，从而及时准确地呈现患者症状表现和康复需求的变化属性和特征； （2）构建口腔癌术后患者居家动态症状管理支持系统，从而为口腔癌术后患者精准生成居家症状管理方案； （3）构建口腔癌术后居家患者精准症状管理模式并应用，从而促进方案的顺利实施，加快患者康复进程。  

Objectives of Study：

This project will establish and implement a precision symptom management model for postoperative oral cancer patients at home based on dynamic patient profiling. Accordingly, the following three research objectives will be pursued:(1) Develop a dynamic profile for postoperative oral cancer patients at home through multidimensional information fusion, enabling timely and accurate representation of the changing attributes and characteristics of patient symptoms and rehabilitation needs;(2) Develop a dynamic symptom management support system for postoperative oral cancer patients at home, enabling the generation of precise home-based symptom management plans tailored to individual patients;(3) Establish and implement a precision symptom management model for postoperative oral cancer patients at home, facilitating smooth plan execution and accelerating patient recovery.

纳入标准：

1.年龄在 18 岁以上； 2.已进行口腔癌根治术并即将出院的患者； 3.预计未来生存期在 6 个月以上； 4.能够自愿接受居家远程症状管理并签署知情同意书

Inclusion criteria

1. Age over 18 years old; 2. Patients who have undergone radical surgery for oral cancer and are about to be discharged; 3. Expected survival period of more than 6 months in the future; 4. Able to voluntarily accept remote home-based symptom management and sign the informed consent form.

排除标准：

1.患有除口腔癌以外的恶性肿瘤或其他严重疾患如心脑血管疾病、呼吸系统疾病等； 2.有精神疾患，无法配合指导者。

Exclusion criteria：

1. Patients with malignant tumors other than oral cancer or other serious conditions such as cardiovascular and cerebrovascular diseases, respiratory diseases, etc. 2. Patients with mental disorders who are unable to cooperate with instructions.

本研究采用标准化数据采集与管理体系，包含两部分： 病例记录表（CRF）：将设计涵盖患者基本信息、口腔癌术后临床病理资料、居家症状动态评估量表（疼痛、进食功能、言语功能、心理状态等维度）、动态患者画像核心数据（生理指标、心理状态、社会支持水平、居家照护能力等）、精准症状管理干预实施记录、随访结局数据（症状缓解率、生活质量评分等）的专属病例记录表。 电子采集和管理系统（EDC）：将依托专业医学数据管理平台搭建电子数据采集系统，实现数据的实时录入、多节点质控、加密存储与溯源追踪，既保障数据安全与隐私合规，也支持动态患者画像的数据分析、迭代更新与可视化呈现，为精准症状管理提供数据支撑。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

A standardized data collection and management system will be adopted in this study, which includes two parts:Case Record Form (CRF): A dedicated CRF will be designed to collect basic patient information, clinicopathological data of oral cancer after surgery, dynamic home symptom assessment scales (covering pain, eating function, speech function, psychological status and other dimensions), core data of dynamic patient portraits (physiological indicators, psychological status, social support level, home care ability, etc.), implementation records of precise symptom management interventions, and follow-up outcome data (symptom relief rate, quality of life score, etc.).Electronic Data Capture (EDC) system: An electronic data capture system will be built based on a professional medical data management platform to realize real-time data entry, multi-node quality control, encrypted storage and traceability tracking. It not only ensures data security and privacy compliance, but also supports data analysis, iterative update and visual presentation of dynamic patient portraits, providing data support for precise symptom management.
