研究目的：

1.明确基于左背外侧前额叶的高频重复经颅磁刺激干预AD患者执行功能的改善效果； 2.比较rTMS治疗前后行为学数据与fMRI大脑网络连接及ERP成分（如P300、N2）的相关性，揭示认知功能改善的潜在神经机制； 3.整合rTMS治疗前后微生物-肠-脑轴多组学数据，验证重复经颅磁刺激改善AD认知功能的间接微生物-肠-脑轴作用。  

Objectives of Study：

1. Clarify the improvement effect of high-frequency repetitive transcranial magnetic stimulation (rTMS) based on the left dorsolateral prefrontal cortex on executive function in AD patients; 2. Compare the correlation between behavioral data before and after rTMS treatment with fMRI brain network connectivity and ERP components (such as P300, N2) to reveal the potential neural mechanisms of cognitive function improvement; 3. Integrate multi-omics data of the microbiota-gut-brain axis before and after rTMS treatment to validate the indirect microbiota-gut-brain axis effect of repetitive transcranial magnetic stimulation on improving cognitive function in AD.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.符合美国国家神经病及语言障碍和卒中研究所—阿尔茨海默病及相关疾病协会（NINCDS-ADRDA）标准中很可能阿尔茨海默病痴呆的临床诊断：报告患者的记忆功能存在逐渐和进行性减退，病程>=6个月，MRI检查显示内侧颞叶萎缩； 2.年龄50-75岁； 3.蒙特利尔认知评估量表（MoCA）评分：10-25分； 4.临床痴呆评定量表（CDR）评分：1-2分； 5.具备良好的视觉、听觉、言语能力，并能配合完成研究需要的神经心理评估和辅助检查； 6.有一位及以上可靠的照料者； 7.患者及/或家属自愿签署知情同意书。

Inclusion criteria

1. Clinical diagnosis of probable Alzheimer's disease dementia according to the National Institute of Neurological and Communicative Disorders and Stroke and the Alzheimer's Disease and Related Disorders Association (NINCDS-ADRDA) criteria: reports of gradual and progressive decline in memory function, disease course >= 6 months, MRI showing medial temporal lobe atrophy; 2. Age 50-75 years; 3. Montreal Cognitive Assessment (MoCA) score: 10-25; 4. Clinical Dementia Rating (CDR) score: 1-2; 5. Good visual, auditory, and speech abilities, and able to cooperate to complete neuropsychological assessments and auxiliary examinations required for the study; 6. At least one reliable caregiver; 7. Patient and/or family voluntarily sign the informed consent form.

排除标准：

1.排除其他导致认知障碍的疾病，如血管性痴呆、额颞叶痴呆等； 2.患有其他中枢神经性疾病，如帕金森病、癫痫等； 3.排除重度精神疾病病史； 4.存在物质滥用或药物依赖史； 5.合并严重心脏、肝肾脏器功能异常者； 6.刺激部位有金属植入； 7.有功能磁共振检查绝对禁忌症。 8.近一个月内连续服用或使用过抗生素（或）微生态调节剂者。

Exclusion criteria：

1. Exclude other diseases that cause cognitive impairment, such as vascular dementia, frontotemporal dementia, etc.; 2. Patients with other central nervous system diseases, such as Parkinson's disease, epilepsy, etc.; 3. Exclude a history of severe mental illness; 4. History of substance abuse or drug dependence; 5. Combined severe dysfunction of the heart, liver, or kidneys; 6. Metal implants at the stimulation site; 7. Absolute contraindications for functional magnetic resonance imaging; 8. Continuous use of antibiotics or probiotics within the past month.
