研究目的：

本研究旨在通过基于健康行为过程取向模型（Health Action Process Approach, HAPA）和助推策略结合构建慢性心衰（chronic heart failure，CHF）合并衰弱患者的太极运动方案，以期为 CHF 合并衰弱患者居家运动康复提供有效方案，并验证其在改善心功能、衰弱、运动效能以及疾病预后等方面的有效性。  

Objectives of Study：

This study aims to develop a Tai Chi exercise program for patients with chronic heart failure (CHF) complicated by frailty, integrating the Health Action Process Approach (HAPA) model and nudge strategies. The program is designed to provide an effective home-based exercise rehabilitation solution for this population and to evaluate its effectiveness in improving cardiac function, frailty, exercise efficacy, and disease prognosis.

纳入标准：

1.年龄≥18岁，符合心力衰竭指南确诊为CHF者，评分纽约心脏病协会(NYHA)心功能分级II级、Ⅲ级者; 2.经治疗后处于疾病稳定期;经Fried衰弱表型筛查为衰弱(满足3条及以上表现); 3.能够独立参与太极运动锻炼，无影响关节运动的肌肉骨骼疾病; 4.拥有1台智能手机。

Inclusion criteria

1. Age >=18 years, diagnosed with CHF according to heart failure guidelines, and classified as New York Heart Association (NYHA) functional class II or III; 2. In a clinically stable phase after treatment, and identified as frail according to the Fried frailty phenotype (meeting >=3 criteria); 3. Physically able to independently participate in Tai Chi exercise, without musculoskeletal disorders affecting joint mobility; 4. Owning a smartphone.

排除标准：

1.3 个月内计划行冠状动脉搭桥手术、经导管主动脉瓣植入术和(或)心脏移植手术等； 2.合并其他器官的严重疾病，如癌症、肝、肾脏衰竭等； 3.无法或不愿意使用远程监护设备(如生活不能自理、痴呆、缺乏沟通能力)； 4.存在严重的骨骼肌肉疾病、骨质疏松或营养不良； 5.出院后计划入住疗养院或其他护理机构； 6.近半年使用太极拳或其他方式规律运动者； 7.正在参与类似或相关研究

Exclusion criteria：

1. Scheduled for procedures such as coronary artery bypass grafting (CABG) surgery, transcatheter aortic valve implantation (TAVI), and/or heart transplantation within the next 3 months; 2. Presence of severe comorbid diseases in other organs, such as cancer, liver or renal failure; 3. Inability or unwillingness to use remote monitoring devices (e.g., severe dependence in activities of daily living (ADL), dementia, lack of communication ability); 4. Presence of severe musculoskeletal disorders, osteoporosis, or malnutrition; 5. Planned admission to a nursing home or other care institution after discharge; 6. Regular exercisers who have practiced Tai Chi or engaged in other forms of structured exercise within the past six months; 7. Current participation in similar or related research studies.
