研究目的：

1．开展临床RCT研究，严格验证体外冲击波治疗（Extracorporeal shock wave therapy，ESWT）作用于小腿三头肌对于下肢运动肌强直缓解的疗效，为其临床转化提供循证医学证据； 2．突破ESWT作用于小腿三头肌用于改善肌强直症状的研究空白，为临床上缓解肌强直方案提供新思路； 3．对比分析ESWT作用于小腿三头肌缓解肌强直症状的有效性，完善ESWT的探头选择、频率、强度等。  

Objectives of Study：

1. Conduct clinical RCT studies to rigorously verify the efficacy of extracorporeal shock wave therapy (ESWT) on the triceps surae in alleviating lower limb spasticity, providing evidence-based medical support for its clinical translation; 2. Break new ground in research on using ESWT on the triceps surae to improve spasticity symptoms, offering new ideas for clinical spasticity management strategies; 3. Compare and analyze the effectiveness of ESWT on the triceps surae in relieving spasticity symptoms, and refine parameters such as probe selection, frequency, and intensity.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）年龄范围18-80岁； （2）诊断符合《中国帕金森病的诊断标准（2016版）》； （3）神志清楚、生命体征稳定的患者； （4）小腿三头肌痉挛，1级≤MAS分级≤3级； （5）经MMSE量表评定大于等于20分，认知能配合试验；

Inclusion criteria

(1) Age range: 18 - 80 years old; (2) Diagnosis conforms to the "Diagnostic Criteria for Parkinson's Disease in China (2016 Edition)"; (3) Clear consciousness, stable vital signs of the patient; (4) Triceps surae spasticity, 1st grade <= MAS grade <= 3rd grade; (5) MMSE assessment score is greater than or equal to 20 points, and cognitive function can cooperate with the test;

排除标准：

（1）有认知障碍、精神障碍的患者； （2）有体外冲击波禁忌症患者（严重凝血功能障碍，治疗区域存在恶性肿瘤）； （3）心、肝、肾、脑功能衰竭等严重合并症的患者； （4）近3个月有下肢手术、踝关节挛缩、严重关节炎等病史者或局部感染及皮肤破溃者； （5）服用抗痉挛药物、局部注射肉毒杆菌毒素、苯酚或酒精等； （6）进行性或者严重的神经系统疾病。

Exclusion criteria：

(1) Patients with cognitive or mental disorders; (2) Patients with contraindications for extracorporeal shock wave therapy (severe coagulation disorders, presence of malignant tumors in the treatment area); (3) Patients with severe comorbidities such as heart, liver, kidney, or brain failure; (4) Patients with a history of lower limb surgery, ankle joint contracture, severe arthritis in the past 3 months, or with local infections or skin lesions; (5) Patients taking antispasmodic medications or receiving local injections of botulinum toxin, phenol, or alcohol; (6) Patients with progressive or severe neurological diseases.
