研究目的：

本研究旨在通过随机对照研究，系统比较保守治疗、单纯半月板后根重建、半月板后根重建结合半月板中心化三种方案，在治疗符合严格纳入标准的中老年半月板后根损伤患者中的短期临床疗效。研究成果将为中老年人群提供治疗策略的高级别循证医学证据，具有重要的临床指导价值和社会经济意义。  

Objectives of Study：

This study aims to systematically compare the short-term clinical efficacy of three treatment regimens - conservative treatment, posterior root reconstruction of the meniscus alone, and posterior root reconstruction combined with meniscus centralization - in treating middle-aged and elderly patients with posterior root meniscus injury who meet strict inclusion criteria through a randomized controlled trial. The research findings will provide high-level evidence-based medical evidence for treatment strategies in the middle-aged and elderly population, and have significant clinical guidance value and social and economic significance.

纳入标准：

(1)患者年龄≥45 岁，≤65 岁； (2)膝关节 Kellgren-Lawrence 0-II 级骨关节炎（OA），无明显膝关节内、外翻畸形或下肢力线异常； (3)查体均可见关节肿胀、关节间隙压痛、关节绞索、麦氏征阳性等一项或多项阳性体征； (4)术前经 MRI 检查明确为半月板后根完全撕裂； (5)术后至少随访 12 个月，随访资料包括量表评分和影像学检查，病例资料完整，包括术前、手术资料和术后随访资料完整； (6)无精神类疾病，能够配合医护人员治疗；

Inclusion criteria

1. Patients aged between 45 and 65 years old; 2. Knee osteoarthritis (OA) of Kellgren-Lawrence grade 0-II, without obvious varus or valgus deformity of the knee joint or abnormal lower limb alignment; 3. Physical examination revealed one or more positive signs such as joint swelling, joint space tenderness, joint locking, and positive McMurray's sign; 4. Preoperative MRI confirmed complete posterior root tear of the meniscus; 5. Postoperative follow-up for at least 12 months, with follow-up data including scale scores and imaging examinations, and complete case data, including preoperative, surgical, and postoperative follow-up data; 6. No mental disorders and able to cooperate with medical staff for treatment.

排除标准：

(1)膝关节 Kellgren-Lawrence III-IV 级 OA，关节不稳定或下肢力线异常（内、外翻畸形>5°）； (2)存在膝关节感染或膝关节其他韧带损伤、膝关节平台及周围骨折等膝关节其他损伤的患者； （3）既往膝关节手术史； （4）存在严重的精神疾病、凝血障碍或其他严重的医疗状况。 （5）具有手术禁忌症或严重影响预后疾病的患者，如年老体弱不能耐受手术者、具有 严重内科疾病者等； （6）不能配合随访，或临床资料不全的患者。

Exclusion criteria：

1. Patients with Kellgren-Lawrence grade III-IV knee osteoarthritis (OA), joint instability or abnormal lower limb alignment (varus or valgus deformity > 5°); 2. Patients with knee joint infection or other knee ligament injuries, knee joint plateau and surrounding fractures or other knee joint injuries; 3. Previous knee surgery history; 4. Patients with severe mental illness, coagulation disorders or other serious medical conditions; 5. Patients with surgical contraindications or diseases that seriously affect prognosis, such as elderly and weak patients who cannot tolerate surgery, patients with severe internal medical diseases,etc; 6. Patients who cannot cooperate with follow-up or have incomplete clinical data.
