研究目的：

1. 以二氧化碳（CO?）激光在口腔黏膜均质型白斑病治疗中的应用为核心，通过单臂研究，系统评估CO?激光治疗该疾病的临床疗效与安全性；观察CO?激光在治疗有效率、复发率等核心临床指标上的差异； 2. 明确CO?激光治疗口腔黏膜均质型白斑病的优势与局限，推动OLK治疗技术的优化升级，为临床中OLK治疗方案的选择提供科学、重要的参考依据。  

Objectives of Study：

1. Focusing on the application of carbon dioxide (CO?) laser in the treatment of homogeneous oral leukoplakia, this study will systematically evaluate the clinical efficacy and safety of CO? laser therapy via a single-arm design, and compare its performance on core clinical indicators including treatment response rate and recurrence rate. 2. This project aims to clarify the advantages and limitations of CO? laser in the treatment of homogeneous oral leukoplakia, promote the optimization and upgrading of OLK treatment technologies, and provide a scientific and important reference for the clinical selection of OLK management strategies.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. 临床以“白色斑块”表现为主，临床诊断为“口腔白斑病”，行组织病理学检查，确诊为“口腔白斑病，不伴有上皮异常增生”； 2. 边界清楚，且病损面积不超过30×30mm^2； 3. 年龄18-70岁，精神及智力状况正常； 4. 生命体征（血压、脉搏、呼吸）无异常者，常规检查（血常规、血糖、凝血功能、乙肝、丙肝、梅毒、HIV检查）无异常者。

Inclusion criteria

1. Patients with clinically diagnosed oral leukoplakia presenting predominantly as white plaques, and pathologically confirmed as oral leukoplakia without epithelial dysplasia by histopathological examination. 2. Lesions with well-defined borders and an area no larger than 30×30 mm^2. 3. Age between 18 and 70 years old, with normal mental and intellectual status. 4. Normal vital signs (blood pressure, pulse, respiration), and normal results in routine examinations including blood routine, blood glucose, coagulation function, hepatitis B, hepatitis C, syphilis, and HIV tests.

排除标准：

1. 孕妇或哺乳期女性； 2. 对阿替卡因局及利多卡因均过敏者； 3. 患有严重全身系统性疾病者（如心血管疾病、糖尿病、肝病、肾功能不全、肿瘤等）； 4. 长期口服避孕药/类固醇的患者； 5. 不适合手术的患者。

Exclusion criteria：

1. Pregnant or lactating women. 2. Patients with hypersensitivity to both articaine and lidocaine. 3. Patients with severe systemic diseases (e.g., cardiovascular diseases, diabetes mellitus, liver diseases, renal insufficiency, tumors, etc.). 4. Patients taking long-term oral contraceptives or steroids. 5. Patients who are not suitable for surgery.

CO2激光治疗：连续脉冲模式，功率10-15W，黏膜-机头距离15-20mm，机头聚焦长度125mm，激光曝光时间20s，光斑直径0.3mm，能量4244 J/cm^2，切取边缘距离病损边缘3mm，深度延伸至临床可见病变以下1-3mm；术后使用醋酸氯己定预防感染，复方甘菊利多卡因凝胶缓解疼痛。

CO2 laser therapy: continuous pulse mode, power 10-15W, mucosa-head distance 15-20mm, head focal length 125mm, laser exposure time 20s, spot diameter 0.3mm, energy 4244 J/cm^2, resection margin 3mm beyond lesion edge, depth extending 1-3mm below visible lesion; postoperative use of chlorhexidine acetate to prevent infection, compound chamomile lidocaine gel for pain relief.
