研究目的：

1、描述 GBM 患者肿瘤组织中小胶质细胞的 TNF-α 表达谱及空间分布特征，明确 TNF-α 高表达型小胶质细胞在肿瘤侵袭前沿的富集情况及其与肿瘤细胞的病理互作模式。 2、分析小胶质细胞中 TNF-α 表达水平高与低的 GBM 患者在人口统计学特征、侵袭表现以及生存预后方面的差异。 3、评估 TNF-α 信号通路激活程度与 GBM 患者对标准放化疗（Stupp 方案）敏感性的关联，并探讨靶向阻断 TNF-α（英夫利昔单抗干预）抑制小胶质细胞介导的胶质瘤侵袭的潜在治疗策略及临床转化意义。  

Objectives of Study：

1. To characterize the TNF-α expression profile and spatial distribution patterns of microglia within the tumor tissues of GBM patients, and to identify the enrichment of TNF-α-high microglia at the invasive front and their pathological interaction patterns with tumor cells. 2. To analyze the differences in demographic characteristics, invasive features, and survival prognosis between GBM patients with high and low TNF-α expression levels in microglia. 3. To evaluate the association between the activation level of the TNF-α signaling pathway and the sensitivity of GBM patients to standard radiochemotherapy (Stupp protocol), and to explore potential therapeutic strategies and clinical translational significance of targeting TNF-α blockade (Infliximab intervention) for inhibiting microglia-mediated glioma invasion.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. 经术后组织病理学确诊为胶质母细胞瘤（WHO 4 级），IDH 野生型（参照 2021 WHO 中枢神经系统肿瘤分类标准）； 2. 术前未接受放疗、化疗或免疫治疗，接受全切手术，术后接受标准放化疗治疗； 3. 临床资料完整（含影像学资料及随访数据）； 4. 于我院生物样本库有样本留存，留存有足够的组织样本用于后续组织病理学染色及分子标记物检测。

Inclusion criteria

1. Histopathologically confirmed glioblastoma (WHO grade 4), IDH-wildtype (according to the 2021 WHO Classification of Tumors of the Central Nervous System); 2. No preoperative radiotherapy, chemotherapy, or immunotherapy; underwent gross total resection followed by standard postoperative radiochemotherapy; 3. Complete clinical data (including imaging profiles and follow-up data); 4. Availability of archived samples in the biobank of our hospital, with sufficient tissue for subsequent histopathological staining and molecular marker detection.

排除标准：

1. 合并其他恶性肿瘤或严重自身免疫性疾病者； 2. 复发性胶质瘤； 3. 围手术期因并发症死亡，缺乏有效随访数据者。

Exclusion criteria：

1. Patients with concurrent other malignant tumors or severe autoimmune diseases; 2. Recurrent glioma; 3. Perioperative death due to complications, resulting in a lack of valid follow-up data.
