研究目的：

本项目通过以接受根治性手术切除的进展期胆囊癌患者为研究对象，通过前瞻性、多中心、非随机同期对照研究方法，根据临床诊疗实际情况非随机化竞争入组，评估腹腔镜和开腹进展期胆囊癌切除术根治效果（R0，1年无瘤生存率），为后续RCT研究提供数据支撑和循证学证据。并探讨开腹胆囊癌根治术与腹腔镜胆囊癌根治术治疗进展期胆囊癌的临床疗效，为临床手术方式选择提供良好证据。  

Objectives of Study：

A prospective, multi-center, non-randomized controlled study was conducted to evaluate the curative effect (R0, 1-year disease-free survival rate) of laparoscopic and open radical resection for advanced gallbladder cancer. To provide data support and evidence-based evidence for subsequent RCT research. To investigate the clinical efficacy of open radical cholecystectomy and laparoscopic radical cholecystectomy in the treatment of advanced gallbladder cancer, and to provide good evidence for the selection of clinical surgical methods.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄＞＝18且＜＝75周岁的中国公民；
2.高度怀疑或经病理证实为胆囊癌肿瘤；
3.经多学科综合评估可接受根治性手术；
4.术前未经过抗肿瘤的放化疗及生物靶向治疗；
5.受试者理解并能配合本研究，签署知情同意书；
6.根据现有循证依据或研究者评估，患者将在此次治疗性手术中获益。

Inclusion criteria

1. Chinese citizens aged ＞＝18 and ＜＝75 years old;
2. Highly suspected or pathologically confirmed gallbladder cancer;
3. Acceptable radical surgery after multidisciplinary comprehensive evaluation;
4. Without preoperative anti-tumor chemoradiotherapy and biological targeted therapy;
5. The subjects understood and could cooperate with this study, and signed the informed consent;
6. On the basis of available evidence or investigator assessment, the patient would benefit from the therapeutic procedure.

排除标准：

1.根据研究者的判断，有严重危害患者安全或影响患者完成研究的伴随疾病；
2.预计生存期小于3个月；
3.不愿意签署知情同意书或不能遵守研究要求者；
4.术前或术中证实肿瘤远处转移患者：腹腔种植，远处淋巴结转移，肝脏或其他脏器转移等；腹部影像资料评估有血管侵犯或术中需联合血管切除的患者；
5.受试者接受姑息性手术（R2）等其他手术方式；
6.经研究者综合判断后认为不适合参与本次研究者。

Exclusion criteria：

1. Concomitant diseases that, according to the investigator's judgment, seriously endanger the patient's safety or prevent the patient from completing the study;
2. The expected survival time is less than 3 months;
3. Unwilling to sign informed consent or unable to comply with the requirements of the study;
4. Patients with distant metastasis confirmed before or during operation: abdominal implantation, distant lymph node metastasis, liver or other organ metastasis, etc. Abdominal imaging data were used to evaluate patients with vascular invasion or intraoperative combined vascular resection.
5. The subjects received palliative surgery (R2) or other surgical methods;
6. According to the comprehensive judgment of the researcher, it is not suitable to participate in this study.
