Antithrombotic consultations encompass various conditions, such as: deep vein thrombosis (DVT) treatment, venous thromboembolism (VTE) prevention, atrial fibrillation, coronary heart disease, cerebral infarction, portal vein thrombosis, and superior mesenteric artery thrombosis.

本文旨在通过回顾性分析湖南医药学院总医院抗栓会诊的病例，探讨抗凝专科药师在抗栓会诊中的作用和效果，评估临床对抗栓会诊的需求及对药师处理综合性临床药学问题能力的变化，评估临床对药师参与抗栓会诊的接受度，呈现临床药师在抗栓治疗中的机遇与挑战，为今后临床药师更好地参与抗栓会诊提供参考和借鉴。  

Objectives of Study：

This retrospective analysis of antithrombotic consultations at Hu'nan University Hospital examines: the role of anticoagulation pharmacists, clinical demand and pharmacist competency evolution, stakeholder acceptance, and practice challenges/opportunities, ultimately informing optimized pharmaceutical care.

纳入标准：

1.临床药师参与的所有以抗栓治疗方案评估或调整为主要会诊目的的会诊患者。具体的子项目中，会限定病种，比如髋关节、膝关节置换术，创伤性脑出血、自发性脑出血等等。

Inclusion criteria

1.This study includes all consultation patients where clinical pharmacists participated primarily for antithrombotic treatment plan evaluation or adjustment. Specific sub-studies will focus on defined conditions, such as total hip/knee arthroplasty, traumatic intracerebral hemorrhage, and spontaneous intracerebral hemorrhage.

排除标准：

1.非临床药师参与的会诊；
2.非抗栓处置为主要会诊目的的会诊；

Exclusion criteria：

1.Those without clinical pharmacist participation;
2.Consultations not primarily for antithrombotic management;
