研究目的：

1.了解糖尿病足患者和其家庭照顾者在照护过程中的体验与需求； 2.制定适用于DF患者的家庭赋权照护方案，探讨其对糖尿病足患者家庭照顾者照顾能力、照顾负担及患者生活质量的干预效果，为DF的家庭照护提供参考。  

Objectives of Study：

1. Understand the experiences and needs of diabetic foot patients and their family caregivers during the care process; 2. Develop a family empowerment care plan suitable for DF patients, and explore its intervention effects on the caregiving ability, caregiving burden, and quality of life of family caregivers of diabetic foot patients, providing references for family care of DF.

纳入标准：

患者的纳入标准：1.符合《中国糖尿病足诊治指南（2024）版》中DF的诊断标准，且Wagner1级及以上的糖尿病足患者；2.年龄＞=18周岁；③患者知情同意，自愿参与访谈；3.具备基本沟通能力和语言表达能力。 照顾者的纳入标准：1.年龄＞=18岁；2.是糖尿病足患者治疗及恢复期间的主要照顾者，平均每日照顾时长＞=4小时，平均每周照顾时间＞=5天，且不收取任何照护费用；3.对研究知情同意、并自愿参与者。

Inclusion criteria

Patients: 1. Meet the diagnostic criteria for diabetic foot (DF) as outlined in the "Guidelines for the Diagnosis and Treatment of Diabetic Foot (2024 edition)" and have Wagner grade 1 or higher diabetic foot; 2. Age >= 18 years; 3. Patient has given informed consent and voluntarily participates in the interview; 4. Possesses basic communication skills and language expression abilities. Caregivers: 1. Age >= 18 years; 2. Is the primary caregiver for the diabetic foot patient during treatment and recovery, averaging at least 4 hours of care per day, with an average of care for at least 5 days per week, and does not charge any caregiving fees; 3. Has provided informed consent for the study and voluntarily participates.

排除标准：

患者的排除标准：1.有严重躯体或心理疾病；2.沟通交流有障碍；3.外伤引起的截肢病史者。 照顾者的排除标准：1.领取劳动报酬的照护者（如护工）接受过照顾技能方面培训的照顾者；3.语言表达障碍的照顾者。

Exclusion criteria：

Patients: 1. Those with severe physical or psychological illnesses; 2. Those with communication barriers; 3. Those with a history of amputation due to trauma. Caregivers: 1. Caregivers who receive a salary (such as nursing assistants); 2. Caregivers who have received training in caregiving skills; 3. Caregivers with speech impediments.

在常规护理的基础上实施家庭赋权方案，根据家庭照顾者的实际情况和照顾需求，灵活调整干预计划；参与者在入组后主要由责任护士以及在责任护士指导下的护理研究生实施。 第一阶段 确定对疾病感知的威胁（介绍家庭赋权的相关知识，明确照顾者的赋权责任、介绍疾病相关知识）；第二阶段 提高自我效能（明确照顾问题、表达情感：了解照顾者存在的负性情绪，缓解照顾者的不良情绪，给照顾者提供情感支持）；第三阶段 教育参与（制定方案、实施方案）；第四阶段 评估结果（干预效果评价、效果追踪评价）

On the basis of routine care, a family empowerment program is implemented, with flexible adjustments to the intervention plan based on the actual situations and care needs of family caregivers; participants are primarily cared for by the responsible nurse and nursing graduates guided by the responsible nurse after enrollment. Phase One: Determine the threat perception of the disease (introducing relevant knowledge about family empowerment, clarifying the empowerment responsibilities of caregivers, and introducing disease-related knowledge); Phase Two: Enhance self-efficacy (clarify caregiving issues, express emotions: understand the negative emotions of caregivers, alleviate caregivers' adverse emotions, and provide emotional support to caregivers); Phase Three: Educational participation (developing plans, implementing plans); Phase Four: Evaluation results (intervention effect evaluation, effect tracking evaluation).

实施常规护理、健康教育及出院指导，内容包括如下：①在患者和照顾者入组时，对其进行一般资料评估，提供护理手册供患者及照顾者自行查阅；②住院期间，由责任护士在每次查房时进行常规口头宣教，内容包括糖尿病足的病因及临床表现、wagner分级、足部护理知识、血糖管理知识、糖尿病用药知识、饮食管理知识、皮肤护理知识、运动及日常生活的注意事项、实验检查的目的、跌倒高风险因素及预防跌倒的方法、局部创面的观察以及常见的不良情绪、不良情绪的缓解方法。及时解答患者及照顾者存在的疑惑，强调正确进行足部护理的意义以及规律控制血糖的重要性。③出院时，对患者及其照顾者进行出院指导，讲解居家护理的注意事项，嘱照顾者监督患者每天按时监测血糖，强调足部监测的重要性，勤观察，如伤口恶化或出现异常，及时就医。④出院后，定期通过电话或微信进行线上随访，随访过程中对研究对象所存在的照护问题进行解答，并叮嘱患者定期复查。

Implement routine nursing care, health education, and discharge guidance, including the following: ① At the time of enrollment, assess general information of the patient and caregiver, and provide a nursing manual for their reference; ② During hospitalization, the responsible nurse provides routine verbal education during each ward round, covering topics such as the causes and clinical manifestations of diabetic foot, Wagner classification, foot care knowledge, blood glucose management, diabetes medication knowledge, dietary management, skin care knowledge, exercise and daily life precautions, the purpose of laboratory tests, high-risk factors for falls and fall prevention methods, local wound observation, common negative emotions, and coping strategies for negative emotions. Promptly answer any questions the patient or caregiver may have, emphasizing the importance of proper foot care and regular blood glucose control; ③ At discharge, provide discharge guidance to the patient and caregiver, explaining precautions for home care, instructing the caregiver to supervise the patient in monitoring blood glucose on time every day, emphasizing the importance of foot monitoring, observing frequently, and seeking medical attention promptly if the wound worsens or abnormalities occur; ④ After discharge, conduct regular follow-ups online via phone or WeChat, during which any care-related issues the patient has are addressed, and remind the patient to have regular check-ups.
