研究目的:

本项目拟结合能谱CT成像理论,重构能谱CT成像网络模型,改善图像质量实现多模态能谱CT图像信息深度耦合,提取脑部血流异常区域特征,结合病理分析特征,进而构建更加合理的急性大血管闭塞血栓来源的人工智能综合预测模型为辅助筛选急性缺血性脑卒中治疗方案提供更客观、更精准、更具普适性的信息,以支持个体化与个性化的临床决策,也为缺血性卒中患者急性管理提供新方向。  

Objectives of Study:

This study aims to integrate spectral CT imaging theory to reconstruct a spectral CT imaging network model, improve image quality, and achieve deep coupling of multimodal spectral CT image information. It will extract features of abnormal cerebral blood flow regions and combine these features with pathological analysis. Subsequently, a more rational artificial intelligence-based comprehensive prediction model for thrombus origin in acute large vessel occlusion will be constructed. This model is intended to provide more objective, accurate, and universally applicable information for assisting in the selection of treatment strategies for acute ischemic stroke, thereby supporting individualized and personalized clinical decision-making. It also offers a new direction for the acute management of ischemic stroke patients.

药物成份或治疗方案详述:

 

Description for medicine or protocol of treatment in detail:

 

纳入标准:

1.年龄18周岁以上;
2.颅内大血管闭塞且行血管内治疗实现再通(mTICI2b-3);
3.发病到完成影像检查时间在4.5小时内;
4.闭塞部位为ICA/MCA M1段;
5.术前能谱NCCT 和能谱CTA影像资料完整;
6.术前能谱NCCT和能谱CTA在同一台CT机上完成且NCCT厚度≤0.25mm、CTA厚度≤0.625mm;
7.对应临床资料(包括但不限于:性别、年龄、是否吸烟、是否患高血压,房颤,冠心病、入院NIHSS、TOAST评分,改良的Rankin量表等)完整的患者;

Inclusion criteria

1.Age 18 years or older; 2.Presence of intracranial large vessel occlusion with successful endovascular treatment achieving recanalization (mTICI 2b?3); 3.Time from symptom onset to completion of imaging examination within 4.5 hours; 4.Occlusion located in the ICA or M1 segment of the MCA; 5.Complete preoperative spectral non?contrast CT (NCCT) and spectral CTA imaging data available; 6.Preoperative spectral NCCT and spectral CTA performed on the same CT scanner, with slice thickness <=0.25?mm for NCCT and <=0.625?mm for CTA; 7.Complete corresponding clinical data (including but not limited to: gender, age, smoking status, history of hypertension, atrial fibrillation, coronary heart disease, admission NIHSS score, TOAST classification, modified Rankin Scale, etc.);

排除标准:

1.合并出血性脑血管病史或有出血倾向者;
2.路径严重迂曲,取栓设备无法到达靶血管者;
3.影像图像非标准的DICOM格式;
4.行EVT治疗后未取出血栓或取出的血栓无法用于病理学研究或患者临床资料获得不全者;

Exclusion criteria:

1.History of hemorrhagic cerebrovascular disease or bleeding tendency; 2.Severe vascular tortuosity preventing thrombectomy devices from reaching the target vessel; 3.Imaging data not in standard DICOM format; 4.Patients from whom thrombus was not retrieved during endovascular thrombectomy (EVT), or retrieved thrombus unsuitable for pathological study, or those with incomplete clinical data.
