研究目的：

本研究通过采用无标记定量蛋白质组学技术对TBM与非TBM患者脑脊液蛋白进行比较分析,筛选出差异蛋白，进一步通过Western blot技术进行候选蛋白验证，筛选出的差异蛋白联合前期研究基础筛选出的13个差异蛋白进行Elisa验证，结合患者临床资料，利用非线性的支持向量机（Support Vector Machine, SVM）分类器分析患者临床资料及筛选出的蛋白，建立诊断模型，评估模型诊断价值，发现相对简便、高效的诊断模型。  

Objectives of Study：

In this study, unmarked quantitative proteomics technology will be employed to compare the protein levels in the cerebrospinal fluid of TBM patients and non-TBM patients. The differentially expressed proteins are identified. Further, candidate proteins will be verified by Western blot technique. The identified differentially expressed proteins are combined with the 13 differentially expressed proteins screened out from the previous study for Elisa verification. Combined with the clinical data of the patients, a non-linear support vector machine (SVM) classifier is used to analyze the clinical data and the screened proteins, and a diagnostic model will be established. The diagnostic value of the model will be evaluated. A relatively simple and efficient diagnostic model will be discovered.

纳入标准：

1.北京胸科医院急诊及住院疑似TBM患者。 2.TBM患者抗结核治疗小于2周，其他中枢神经系统感染患者治疗小于1周。 3.无腰椎穿刺禁忌症 4.患者同意入组，并签署知情同意书。

Inclusion criteria

1. Emergency and inpatient patients with suspected TBM at Beijing Chest Hospital. 2.Patients with TBM who have received anti-tuberculosis treatment for less than 2 weeks, and patients with other central nervous system infections who have received treatment for less than 1 week. 3. There are no contraindications for lumbar puncture. 4. The patients have agreed to participate in the study and have signed the informed consent form.

排除标准：

1.诊断为可能结核性脑膜炎患者。 2.不能明确诊断患者。 3.脑外伤或颅内有创检查导致颅内感染者。 4.长期使用糖皮质激素、非甾体抗炎药，或免疫抑制剂。

Exclusion criteria：

1. Patients diagnosed with possible tuberculous meningitis. 2. Patients whose diagnosis is unclear. 3. Infections in the brain caused by brain trauma or invasive intracranial examinations. 4. Long-term use of glucocorticoids, non-steroidal anti-inflammatory drugs, or immunosuppressants.

金标准或参考标准（即可准确诊断某疾病的单项方法或多项联合方法，在本研究中用于诊断是否有该病的临床参考标准）：

结核性脑膜炎诊断标准：按照国际结核性脑膜炎诊断标准分为确诊、很可能、可能。 确诊TBM需要符合如下各项要求。①符合至少1项以下临床表现：头痛、易激惹、呕吐、发热、颈强直、抽搐、局部神经损害、意识障碍、昏睡，同时具备以下1项或多项：脑脊液（CSF）中查到抗酸杆菌；CSF中分枝杆菌培养阳性；CSF中结核分枝杆菌核酸检测阳性。②脑或脊髓中发现结核性病理改变，抗酸染色阳性，同时有临床表现和相应的CSF改变。 很可能TBM：符合上述至少1项临床表现；同时具备：临床评分10分以上（未进行颅脑影像学检查）；或临床评分12分以上（有颅脑影像学检查）；CSF或影像学评分至少2分；排除其他脑膜炎。 可能TBM：有头颅CT或头颅MRI检查患者，临床评分6-11分；无头颅CT或头颅MRI检查患者，临床评分6-9分，同时除外其他疾病。如果头颅CT、头颅MRI检查、腰椎穿刺均未做，则不能诊断或除外TBM。 细菌性脑膜炎、真菌性脑膜炎、病毒性脑膜炎、脑膜恶性肿瘤、系统性红斑狼疮脑炎、自身免疫性脑炎等按照人卫版《神经病学》第九版诊断标准执行。

Gold Standard or Reference Standard (The clinical reference standards required to establish the presence or absence of the target condition in the tested population in present study):

Diagnostic criteria for tuberculous meningitis: According to the international diagnostic criteria for tuberculous meningitis, it is classified as confirmed, highly likely, and possible. Confirmed TBM: The following requirements must be met: 1. At least one of the following clinical manifestations must be present: headache, irritability, vomiting, fever, neck stiffness, convulsions, local neurological damage, altered consciousness, coma, and at least one or more of the following: acid-fast bacilli detected in cerebrospinal fluid (CSF); positive mycobacterium culture in CSF; positive nucleic acid test for Mycobacterium tuberculosis in CSF. 2. Tuberculous pathological changes are found in the brain or spinal cord, acid-fast staining is positive, and there are clinical manifestations and corresponding CSF changes. Highly likely TBM: (1) At least one of the above clinical manifestations is present; (2) A clinical score of more than 10 points (no cranial imaging examination); or a clinical score of more than 12 points (with cranial imaging examination); (3) At least 2 points in CSF or imaging scores; (4) Exclusion of other meningitis. Possible TBM: (1) For patients with head CT or head MRI examination, the clinical score is 6-11 points; (2) For those without head CT or head MRI examination, the clinical score is 6-9 points, and at the same time, other diseases are excluded. Note: If head CT, head MRI examination, and lumbar puncture have not been performed, TBM cannot be diagnosed or excluded. Other meningitis types: Bacterial meningitis, fungal meningitis, viral meningitis, malignant meningitis, systemic lupus erythematosus encephalitis, autoimmune encephalitis, etc. follow the diagnostic criteria of the 9th edition of "Neurology" by People's Medical Publishing House.
