研究目的：

引入可穿戴设备，搭建高效、实用的院内外物联网一体化智慧健康监测平台，设计清晰、全面的患者健康报告，实现慢性病患者健康数据的实时采集、传输、分析、风险预警和可视化综合健康报告展示。  

Objectives of Study：

Introduce wearable devices, establish an efficient and practical integrated IoT smart health monitoring platform both inside and outside the hospital, design clear and comprehensive patient health reports, and achieve real-time collection, transmission, analysis, risk warning, and visual display of comprehensive health reports for patients with chronic diseases.

药物成份或治疗方案详述：

将通过可穿戴设备收集测试样本的健康数据，进行（1）接口调试：验证患者佩戴的可穿戴设备能否稳定、准确地将患者的健康数据（如心率、血压、血氧、活动步数等）传输至医院的医疗平台。（2）报告优化：根据初步健康报告的生成，优化报告的内容布局、图表清晰度和信息可读性，使其更直观、易懂，便于患者和医护理解。 

Description for medicine or protocol of treatment in detail：

Health data from test samples collected via wearable devices will be used for (1) interface debugging: verifying whether the wearable devices worn by patients can stably and accurately transmit their health data (such as heart rate, blood pressure, blood oxygen, activity steps, etc.) to the hospital's medical platform. (2) report optimization: based on the generation of preliminary health reports, optimizing the content layout, chart clarity, and information readability of the reports to make them more intuitive, understandable, and convenient for patients and medical staff to comprehend. 

纳入标准：

1.患者符合世界卫生组织（WHO）制定的慢性病诊断标准； 2.年龄≥18岁； 3.意识清楚，能够理解问卷内容，非文盲。

Inclusion criteria

1.The patient meets the diagnostic criteria for chronic diseases established by the World Health Organization (WHO); 2. Age >= 18 years old; 3.Have clear consciousness, be able to understand the content of the questionnaire, and not be illiterate.

排除标准：

1.听力、视觉、视力、精神障碍病人； 2.合并重要器官功能障碍； 3.卧病在床不能行动； 4.近1个月内发生影响情绪的重大个人或家庭事件。

Exclusion criteria：

1.Patients with hearing, visual, visual acuity, and mental disorders; 2.Combined with significant organ dysfunction; 3.Being bedridden and unable to move; 4.Significant personal or family events that have affected your mood within the past month.
