研究目的:

本研究探讨甲状腺结节超声特征(灰阶超声、弹性成像及微血流定量分析)与颈部淋巴结转移的相关性;聚焦于颈部淋巴结,筛选弹性成像(剪切波弹性及粘弹性)、微血流分析(HoloUMA)诊断甲状腺乳头状癌颈部淋巴结转移的定量参数,并分析甲状腺乳头状癌淋巴结转移相关参数的阈值;再将甲状腺结节参数、淋巴结参数结合在一起,建立一个多模态超声参数(包括灰阶超声、HoloUMA、弹性成像)联合临床特征的诊断模型,为PTC淋巴结转移提供无创、高灵敏度的联合诊断方法, 优化临床决策,减少不必要的淋巴结活检或手术。  

Objectives of Study:

This study investigates the correlation between the ultrasonic characteristics of thyroid nodules (including grayscale ultrasound, elastography, and quantitative microvascular flow analysis) and cervical lymph node metastasis. It focuses on cervical lymph nodes to identify quantitative parameters from elastography (shear wave elasticity and viscoelasticity) and microvascular flow analysis (HoloUMA) for diagnosing cervical lymph node metastasis in Papillary Thyroid Carcinoma (PTC), and to determine the thresholds for these parameters associated with PTC metastasis.Subsequently, parameters from both thyroid nodules and lymph nodes will be integrated to establish a diagnostic model. This model will combine multimodal ultrasonic parameters (including grayscale ultrasound, HoloUMA, and elastography) with clinical features. The aim is to provide a non-invasive and highly sensitive combined diagnostic method for PTC lymph node metastasis, optimize clinical decision-making, and reduce unnecessary lymph node biopsies or surgeries.

药物成份或治疗方案详述:

 

Description for medicine or protocol of treatment in detail:

 

纳入标准:

1.年龄大于 18 周岁;
2.诊断有可疑甲状腺乳头状癌结节,至少在 VI 区发现淋巴结,伴或不伴侧颈部发现可疑淋巴结;
3.获得满意的目标结节及常规超声、弹性成像、 HoloUMA 图像;
4.拟于本院接受甲状腺及淋巴结 FNA 穿刺及病理检测,以及后续手术干预的患者。

Inclusion criteria

1. Age greater than 18 years; 2. Presence of suspicious papillary thyroid carcinoma nodule(s) with at least one lymph node detected in level VI, with or without suspicious lymph nodes in the lateral neck; 3. Availability of satisfactory images of the target nodule obtained from conventional ultrasound, elastography, and HoloUMA; 4. Confirmed diagnosis of papillary thyroid carcinoma in the thyroid nodule by FNA or histopathology, and confirmed pathological results (by FNA or surgical excision) for all suspicious lymph nodes.

排除标准:

1.未在本院行手术/FNA 导致资料不完整者;
2.无明确的病理诊断结果者;
3.合并其他部位恶性肿瘤者;
4.甲状腺结节消融病史者;
5.有颈部放射史,或有甲状腺癌家族史的患者。

Exclusion criteria:

1. Patients with incomplete clinical data due to not undergoing surgery/FNA at our hospital; 2. Patients without a definitive pathological diagnosis; 3. Patients with concurrent malignancy in other organs; 4. Patients with a history of thyroid nodule ablation; 5. Patients with a history of neck irradiation or a family history of thyroid cancer.

数据采集和管理(说明:数据采集和管理由两部分组成,一为病例记录表(Case Record Form, CRF),二为电子采集和管理系统(Electronic Data Capture, EDC),如ResMan即为一种基于互联网的EDC:

数据采集:均应用同一超声仪器型号,由经过培训的超声医师进行图像采集,并由PI单位进行质量控制; 数据管理:本研究构建了一套完整的数据管理方案:采用标准化的电子病例报告表(e-CRF)及数据字典统一采集多模态超声与病理数据;通过集中培训、双人录入、实时逻辑核查与定期质控确保数据准确性;依托 DICOM 标准中央库管理影像, 硬盘采用加密处理,并由多名专家盲法评估;所有数据经匿名化处理,实行权限控制、加密传输及异地备份,全面保障数据安全与合规。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture:

Data collection: The same ultrasound instrument model was used for all examinations. Image acquisition was performed by trained sonographers, with quality control conducted by the principal investigator (PI) site.Data Management:This study established a comprehensive data management framework: A standardized electronic case report form (e-CRF) with a corresponding data dictionary was employed to uniformly collect multimodal ultrasound and pathological data. Data accuracy was ensured through centralized training, double-data entry, real-time logic checks, and regular quality control audits. Images were managed relying on a central repository adhering to the DICOM standard. All hard drives were encrypted, and images were evaluated blindly by multiple experts. To comprehensively ensure data security and compliance, all data were anonymized, with implemented access controls, encrypted transmission, and off-site backups.
