研究目的：

本研究针对老年患者口腔舒适化治疗镇痛药物选择这一关键问题，拟以接受多颗牙拔除的老年患者为研究对象： （1）系统研究瑞马唑仑联合不同剂量阿芬太尼镇静过程中呼吸抑制发生率； （2）探讨不同剂量组临床应用的有效性、安全性等。  

Objectives of Study：

This study addresses the critical issue of analgesic selection for comfortable dental treatment in elderly patients. It plans to enroll elderly patients undergoing multiple tooth extractions as study subjects to: (1) systematically investigate the incidence of respiratory depression during sedation with remimazolam combined with different doses of alfentanil; (2) evaluate the clinical efficacy, safety, and other outcomes across different dose groups.

纳入标准：

1.空军军医大学第三附属医院接受牙拔除术患者 2.年龄60~80周岁 3.ASA分级Ⅰ~Ⅲ级 4.体重18~30kg/m^2 5.拔牙颗数>=3颗 6.患者同意接受该研究

Inclusion criteria

1. Patients undergoing dental extraction at the Third Affiliated Hospital of the Air Force Medical University 2.Age 60~80 years old 3. ASA classification Ⅰ ~ Ⅲ 4. Weight 18~30kg/m^2 5. Number of extracted teeth >= 3 teeth 6. Patient's consent to the study

排除标准：

1.神经系统疾病史（认知障碍、脑损伤等） 2.严重心脑血管系统疾病 3.肝肾功能损伤或急性肝肾功能衰竭 4.已知对镇静或局麻药物过敏 5.阿片类药物滥用史 6.牙周急性炎症期，不适合进行牙拔除术

Exclusion criteria：

1. History of neurological disorders (cognitive impairment, brain injury, etc.) 2. Severe cardiovascular and cerebrovascular system diseases 3. Hepatic or renal impairment or acute hepatic or renal failure 4. Known hypersensitivity to sedative or local anaesthetic drugs 5. History of opioid abuse 6. Acute periodontal inflammatory period, unsuitable for dental extractions

Blinding：

A designated study personnel prepared the corresponding study drugs (all drugs were clear, unlabeled, and identical in volume per group). The anesthesiologist was blinded to group allocation and was only responsible for collecting patient data. Statistical analysis was conducted by a dedicated statistician who was also blinded to group assignment.
