研究目的：

1.探索利用床旁肺部超声的方式引导儿童急性呼吸窘迫综合征肺复张治疗； 2.对比肺部超声法引导肺复张与最大氧合法指导肺复张的效果。  

Objectives of Study：

1.To investigate the application of bedside ultrasound in guiding lung recruitment maneuvers (RM) in pediatric acute respiratory distress syndrome (PARDS). 2. To compare the efficacy of bedside ultrasound-guided lung RM (US) with that of maximal oxygen-guided lung RM (OXY) in the management of PARDS.

纳入标准：

1.年龄范围：4周~14岁； 2.符合PALICC关于PARDS的定义； 3.机械通气持续时间<72小时； 4.PEEP水平在5～15cmH2O之间。

Inclusion criteria

1.The age range is 0-14 years old; 2.Meeting the Pediatric Acute Lung Injury Consensus Conference (PALICC) definition of Pediatric Acute Respiratory Distress Syndrome (PARDS). 3.Duration of mechanical ventilation <72 hours; 4.PEEP level between 5 and 15cmH2O.

排除标准：

1.符合PARDS诊断标准但机械通气时间超过72小时； 2.血流动力学不稳定，严重左心和右心功能障碍； 3.近6小时内调整血管活性药物剂量幅度超过50%； 4.血乳酸>2mmol/L； 5.毛细支气管炎、先天性膈疝、限制性肺病、肺纤维化； 6.持续吸入NO或口服西地那非； 7.重型颅脑损伤； 8.使用体外膜肺生命支持。

Exclusion criteria：

1. Meet the diagnostic criteria of PARDS but mechanical ventilation for more than 72 hours; 2. Hemodynamic instability with severe left and right heart dysfunction; 3. The dose of vasoactive drugs was adjusted by more than 50% within the last 6 hours; 4. Blood lactic acid >2mmol/L; 5. Bronchiolitis, congenital diaphragmatic hernia, restrictive lung disease, pulmonary fibrosis; 6. Continuous inhalation of NO or oral sildenafil; 7. Severe traumatic brain injury; 8. Use extracorporeal membrane lung life support.

借鉴波士顿儿童医院Wolf GK的方案对纳入的患儿进行肺复张治疗，整个肺复张过程分2个步骤：基线段和肺复张段，舍去持续性肺膨胀治疗。机械通气基线段采用容量控制通气模式进行低流速送气，设置潮气量为6ml/kg（标准体重），观察P-V曲线，找到低位拐点。 肺复张段采用压力控制通气模式，以PEEP=15cmH20、PPlat=30cmH20作为肺复张治疗的起始，此时在深度镇静和肌松状态下将呼吸频率降至20~25次/分，吸气时间增至1.2秒，FiO2维持在100%，持续2min后调整参数，PEEP与PPlat同时上调5cmH20，然后再保持3min。每次调整参数稳定2分钟后进行肺部超声检查及血气分析，判断肺复张的程度与充分性。若PPlat达到50cmH20仍未能复张，需停止肺复张操作。

干预措施代码：

Intervention：

The initial phase of mechanical ventilation employs a volume control ventilation mode to deliver air at a low flow rate, with a tidal volume set at 6 ml/kg of ideal body weight. The pressure-volume (P-V) curve should be examined to identify the lower inflection point. The pressure-controlled ventilation mode is adopted in the lung recruitment section. PEEP=15cmH2O and PPlat=30cmH2O are used as the starting point of lung recruitment treatment. At this time, the respiratory rate is reduced to 20~25 times/min under the condition of deep sedation and muscle relaxation, the inspiratory time is increased to 1.2 seconds, FiO2 is maintained at 100%, and the parameters are adjusted after 2 minutes. PEEP and PPlat are both up-regulated at 5cmH2O and then maintained for 3 minutes. During this period, blood gas analysis and lung ultrasound are performed to determine the degree and sufficiency of lung recruitment. If PPlat reaches 50cmH2O but still cannot be reopened, the lung reopening operation needs to be stopped.
